I’m Dr. Tiffany Parks — an internal medicine trained physician. I practice menopause and midlife care because this is where I saw women being dismissed most often.

The stories were always some version of the same thing. Symptoms that were real. Visits that were short. And an explanation that arrived before anyone had really looked — you’re stressed, you’re tired, this is just your age.

Sometimes stress is part of it. Sometimes age is part of it. But “part of it” is not the same as “all of it.” And that difference matters enormously when you’re the one living in your body.

So this is how I practice: I start by listening. I take a full history. I look at what’s actually going on — hormones, thyroid, sleep, metabolic health, mental health, medications — before deciding what may be contributing.

Sometimes menopause is part of the answer. Sometimes it’s something else entirely that was missed while everyone was busy calling it menopause.

That’s what midlife care, done right, looks like: unhurried, thorough, and evidence-based.

It’s what I’d want for myself.

The word was right there. Then it wasn’t — mid-sentence, in front of everyone.

Brain fog is one of the most common concerns women bring to me in perimenopause and menopause, and one of the most quietly frightening. Losing words. Walking into a room and forgetting why. Reading the same paragraph three times. High-performing women start to wonder if something is seriously wrong.

Here’s what I tell every woman who asks: this is common, for most women it lifts, and it is very rarely early dementia. That’s not a reason to ignore it — it’s a reason to look properly.

Stress is real, and it can be part of this. But “it’s just stress” usually means no one looked any further.

Brain fog in midlife can have more than one driver. Sleep can feed it. Mood can feed it. Hormonal shifts can play a role. That’s why a real evaluation looks at your symptoms, your sleep, your mood, your medications, and labs — thyroid, iron, B12 — because more than one thing can be driving this.

Three things worth doing while you sort it out. Guard your sleep the way you'd guard a meeting you can't miss. Write it down instead of asking a tired brain to hold it. And keep a short note of when the fog is worst — after a bad night, at a certain point in your cycle, at a certain hour of the day. That pattern is the most useful thing you can bring to a visit.

“What actually happens when I book?”

Fair question.

Here’s the honest answer.

Before we ever meet, you fill out a short intake — about five minutes.

Your symptoms, your history, what’s been going on.

I review all of it before we meet.

So when you log on, you’re not starting from zero with a stranger. I already know the outline of your story. Now I want to hear it in your words.

Then you talk.

That’s not a courtesy — it’s the method.
What’s bothering you most. What you’ve already tried. What your days actually look like.

From there, we go through the whole picture together: your medical history, sleep, nutrition and exercise, cardiovascular risk, bone health, family history, stress, and mood — because midlife symptoms rarely travel alone, and understanding how they fit together can change the plan.

Then we decide together.

Whether hormone therapy makes sense for you. Whether a GLP-1 is appropriate if weight is part of your picture. Non-hormonal options, if that’s your preference.

The plan is built around your history — not a formula, and not a script.

Before we hang up, your follow-up is on the books. If medication is part of the plan, your prescription is sent to your pharmacy or arranged for home delivery.

After the visit, you get a written summary of what we discussed and your individualized plan.

“Just turn the thermostat down.”

By the time a woman mentions night sweats to me, she has usually already tried that. And the fan, and the lighter blanket, and sleeping on top of the covers with one leg out.

What she’s describing isn’t being warm. It’s the heat waking her at two in the morning, hours before the alarm — changing her shirt in the dark, lying back down with her heart racing, calculating how much night is left.

Then she gets up and runs her day on that.

She’ll often tell me about it almost apologetically, tacked onto the end of a visit that was supposed to be about something else — because it sounds like a complaint about temperature rather than a symptom. Meanwhile the exhaustion it causes gets treated as a sleep problem, and the short fuse that comes from running on four broken hours gets treated as stress.

Vasomotor symptoms are a common feature of the menopause transition — and among the most treatable. There are real options, and which one fits depends on your history, not on a formula.

You shouldn’t have to convince someone that something has changed.

Many women I meet have already told the story more than once. To a friend. To a doctor. Sometimes to two or three. By the time they reach me, they’ve started editing it down — leaving out the parts that got a blank look last time, bracing for the moment someone says it’s stress.

That editing is a problem, and not only because it’s exhausting. The details that get dropped are often the useful ones. When your energy changed. What wakes you at night. Whether your cycle shifted before anything else did. That’s the material an evaluation is built from.

So the first visit here is longer than you may be used to, and a significant part of it is you talking.

Not because I’m being generous with time. Because I can’t help you without it.

Getting a prescription isn’t the same thing as being cared for.

A prescription can arrive at your door in days. But who is watching what happens after you start it?

Hormone and metabolic therapy isn’t “send it and forget it” medicine. Your response matters. Your symptoms matter. Side effects matter. And when something changes, the plan may need to change with you.

Too much of modern healthcare has made access to the prescription the finish line.

I don’t believe it is.
