Here’s what a lot of people don’t realize: a “normal” or unremarkable scan doesn’t mean nothing is going on.

Research has shown that imaging changes in the spine — even ones that look concerning — show up all the time in people who have zero pain.

And the reverse is true too: plenty of people in real pain have scans that look pretty unremarkable. Imaging alone often can’t tell the whole story of what you’re feeling. 📋

A landmark placebo-controlled trial (the OPAL trial, published in The Lancet) found opioids were no more effective than placebo for acute low back and neck pain — and came with real risks of misuse down the line.

Federal regulators have approved a tablet that combines two nonprescription pain relievers, “expanding non-opioid options for persistent pain,” manufacturer Kenvue Brands said in a release.

The U.S. Food and Drug Administration announced July 24 that it has approved Tylenol with Naproxen, “the first fixed-dose combination product to pair acetaminophen with naproxen sodium in a single combination tablet.”

For decades we assumed opioids must work for back pain - they’re powertul painkillers, after all. But when researchers finally put that assumption to the test, the results were humbling.

In the placebo-controlled OPAL trial (published in The Lancet), a short course of opioids relieved acute back pain no better than a placebo - and caused more side effects like constipation.

It’s one trial, and it studied acute pain —but it removed the strongest reason to reach for opioids early.

“Shouldn’t I just get an MRI?”

Scans are great at finding something. They’re not always great at finding the cause.

Disc “degeneration” shows up in tons of people who have zero back pain — 37% in their 20s, all the way up to 96% in their 80s. It’s often just… aging. Not damage. Not the reason you’re hurting.

That doesn’t mean imaging is useless — it means a scan alone can’t tell your whole story. Pain is complicated, and treatment should be too.
