What happens to the face and neck after significant weight loss?

This patient put in the hard work and lost a considerable amount of weight, and the change left her looking older than her years. She came to us wanting to bring her face back in line with how she felt.

Dr. Weinfeld performed a deep plane facelift and a deep plane neck lift. The facelift elevated the cheeks and softened the jowls. The neck lift addressed the loose skin and bulk through the submandibular and submental region, with visible results.

Hers was a preservation deep plane neck lift. In her case, the skin was not elevated off the platysma muscle. Instead, that muscle was repositioned and used to support the skin lying over it. That differs from a dual plane neck lift, which is the approach when there is a significant amount of excess skin that also needs to be elevated and redistributed.

What does "shape over size" actually look like on the table?

Dr. Reis walks through a breast augmentation from first markings to final result. Our patient is in her late 30s, a mom of two, with a little bit of ptosis (the natural descent of breast tissue that can happen over time). What she wanted was better cleavage, more fullness and projection, and a size that suits her frame rather than a number.

Why does menopause seem to move weight to the abdomen and take it from everywhere else?

Dr. Reis is answering a question that comes up constantly, and the framing matters here. Menopause does not necessarily cause weight gain. What it causes is weight redistribution.

πŸ”„ It starts with hormones. During menopause the ovaries stop producing estrogen, progesterone, and even testosterone. Estrogen is the hormone responsible for feminization, which includes storing fat in the breasts, hips, glutes, and thighs.

πŸ“ So the fat relocates. As estrogen levels fall, the tendency to store fat in those areas decreases. That fat still has to go somewhere, and it tends to land in the abdomen, both as intra abdominal fat and as fat under the skin.

πŸ’Š One option is hormone replacement therapy. Estrogen, progesterone, and testosterone can all be replaced, and the right approach depends entirely on your specific needs. Worth being clear though: hormone therapy is not a weight loss solution on its own. It is not thermogenic and it does not change your appetite.

πŸ’ͺ What it actually does. Testosterone in particular supports better sleep and better energy levels, and it helps you build lean muscle mass much more quickly. That can lead to a real recomposition of your body and a shift in overall fat distribution.

The bottom line from Dr. Reis: there is nothing wrong with your body during menopause. It simply means the strategies that worked in your 20s and 30s are not the strategies that work in your 50s and 60s.

What happens when a mini facelift isn't enough?

This patient had one years ago. Four months out from a full facial rejuvenation with Dr. Weinfeld, here is where she is now.

The foundation was a deep plane face and neck lift with structural work in the neck: submandibular gland reduction, reduction of the anterior belly of the digastric, and platysmaplasty centrally and laterally. Fat grafting added support to the cheeks and temples. She also had a lip lift, a hidden scar upper facelift with an endoscopic brow lift, and upper and lower lid blepharoplasty.
