*Paxlovid is dispensed in office , so no more looking for a pharmacy that carries it. We have it in stock!

Home of the 10$ Youth/HS Sports Physical for
Moreno Valley, Perris, and Banning.
Please bring all necessary forms to fill out at time of physical. If you don't have your forms, please contact the organization or school to make arrangements to get them before being seen here in our offices.
To help maintain the low price it's important Athletes/Parents are aware what is needed, and come in prepared.
-Due to increase of wrong forms being given there will be a 10$ redo fee for all physicals that need to be rewritten on another form
-Replacement or Copy of Sports Physicals will have a 10$
