💙 Kirsten Lawson, RN, BSN

Sabrina and Kirsten have joined our growing team as Chronic Care Managers, bringing experience, compassion, and a shared commitment to improving the health and well-being of our patients.

Living with a chronic condition such as diabetes, high blood pressure, heart disease, COPD, asthma, or other long-term illnesses can be challenging—but no one should have to manage it alone. Our Chronic Care Managers work one-on-one with patients to coordinate care, provide education, monitor progress, assist with medications, connect patients with needed resources, and help them stay on track with their healthcare goals.

By building trusting relationships and maintaining regular communication, they empower patients to better understand their health, prevent complications, reduce hospital visits, and achieve better long-term outcomes. Their support extends beyond appointments, ensuring our patients always have someone advocating for their health.

At Moore Care Clinic, we continue to grow because our community deserves exceptional healthcare close to home. We are proud to welcome Sabrina and Kirsten to our ever-expanding team and look forward to the positive impact they will make in the lives of our patients.

Moore Care Clinic has had a busy and rewarding week serving our community! 💙💚

Yesterday afternoon and evening, our amazing team had the opportunity to attend the Lee County Elementary, Middle, and High School Open Houses, where we enjoyed meeting so many students, parents, teachers, and community members.

It is always a pleasure to answer questions about our healthcare services, share information about healthy habits, and help families prepare for a successful and healthy school year. Building relationships within our community is at the heart of everything we do, and we are grateful for every conversation and every smiling face we met along the way.

Thank you to the Lee County School District for inviting us to be part of these wonderful events. We look forward to continuing to serve our community with compassionate medical and behavioral health care.
