Week 2 Update โ€” More than just a knee ๐Ÿฆต

Recovery doesn't stop at the surgery site.

This week we introduced shockwave therapy to help reduce inflammation, improve circulation, and support tissue healing around the surgical area. Since my steri-strips are still on, we applied it to the back of the knee to protect the repair, and I noticed less swelling and easier movement for the rest of the day.

One thing I've really appreciated about this phase of recovery is that my team at Bellingham Physical Therapy isn't just focused on my knee. With so much more sedentary time than I'm used to, it's been important to keep the rest of my body moving too, working on general movement, core strength, back, and even my opposite leg to help offset the effects of walking in a locked brace for weeks.

As a PT, this is exactly how I believe care should work. Treating the whole person, not just the injury.

One thing Iโ€™m learning quickly during recovery is that what happens between PT appointments matters just as much as what happens during them.

Iโ€™ve been using an icing and compression machine at home to help manage swelling and pain between exercise and activity sessions. Unfortunately these devices are typically not covered by insurance, but Iโ€™ve found it really helpful for improving comfort and allowing me to participate more successfully in both PT and my home exercise program.

My PT, Erica Anderson, DPT, also set me up with a home program through the Bellingham Physical Therapy app. The setup process was simple, and Iโ€™ve appreciated being able to follow videos and individualized instructions specific to my recovery.

The app also keeps me accountable between visits because Erica can see whether Iโ€™ve completed my exercisesโ€ฆ so apparently thereโ€™s no skipping homework ๐Ÿ˜…

One thing I appreciate as both a PT and now a patient is how technology can help improve communication and consistency during recovery. If questions come up between appointments, I can even message my provider directly through the app.

During todayโ€™s session, we also combined NMES with blood flow restriction training at the same time to improve quad activation and help maintain muscle during the early protection phase after surgery.

Recovery still feels very early, but small wins already feel meaningful.

Last Friday was my first PT session after ACL repair surgery โœ”๏ธ

One thing that surprised me already as a patient is how quickly rehab starts after surgery. During the session, we looked at my range of motion and immediately started working on quad activation to help prevent as much muscle atrophy as possible while Iโ€™m still using the brace to protect the repair.

We used two different tools during treatment:
โ€ข Neuromuscular electrical stimulation (NMES) to help improve quad contraction
โ€ข Blood flow restriction (BFR) training to help signal my body to maintain and build muscle rather than break it down, which commonly happens after trauma or surgery

By the end of the session, I was already able to get a stronger muscle contraction on my own with less pain.

As a PT, these are techniques I use with patients regularly. Itโ€™s interesting experiencing them from the other side of the table now.

Recovery still feels very early and humbling, but it also feels good to already be moving and working toward rebuilding strength.

Recoveries usually happen behind closed doors, but I thought it might be meaningful to share a little of mine.

Last Wednesday I underwent ACL repair surgery using the BEARยฎ Implant procedure, a newer technique designed to help the body heal its own ACL rather than requiring a graft harvest from elsewhere in the body.

These photos were taken about 6 hours post-op while navigating the pain meds, brace, swelling, and the reality of those first few recovery hours. I had already started some post-op exercises by then and was quickly learning that it feels very different being the patient instead of the PT.

One thing this experience has already reminded me is how much happens behind the scenes in patient care. Just days before surgery, insurance initially denied pre-authorization for the procedure. Thankfully, Ariel Turner, PA-C at Pacific Rim Orthopedic Surgeons participated in a peer-to-peer review with the insurance utilization management team and helped get approval so care could move forward.

At Bellingham Physical Therapy, we regularly advocate for patients in similar ways because continuity of care matters.

As both a physical therapist and now a patient, I thought it could be interesting to share parts of the recovery process from both perspectives. The challenges, milestones, rehab progression, and what physical therapy really looks like after surgery.
