When a patient tells me their GLP-1 “isn't working,” the scale is usually the starting point.

But an unchanging number doesn't always give us enough information to judge the treatment.

The scale is one piece of data. If weight has stalled while other factors are changing, I widen the lens before changing the plan.

That's how we decide what needs attention next.

The number on the scale can be useful information but its not the absolute verdict.

I care about weight because it can tell us something about what's happening metabolically. But I also care about what happens to blood pressure, blood sugar, fitness, strength, sleep and daily function.

Sometimes the scale changes before you notice those improvements. Sometimes those improvements happen without much change on the scale.

The goal is to use the information that helps us make better decisions — and stop giving one number the power to decide whether you're succeeding.

If you expect your weight to go down every single week, you're going to think you're failing a lot more often than you actually are.

Weight loss is usually much messier than the straight downward line we imagine.

There can be plateaus.
There can be fluctuations.
There can be weeks where the scale goes up.

The question is whether the overall trend is moving in the direction you want.

This is especially important when you're using a GLP-1. Don't let one frustrating number convince you that your treatment has stopped working.

Zoom out and look at the trend.

There was a version of my career where I stayed in the traditional insurance-based system.

I wanted it to work.

But I couldn't practice medicine the way I believed my patients deserved.

When you're given 15 minutes, you're expected to diagnose, prescribe, document and move on. That might work for some problems but it doesn't work for obesity.

You can't unpack years of weight cycling, family influences, cultural traditions, emotional eating, changing metabolism or the realities of daily life in 15 minutes.

Most importantly, you can't build trust in 15 minutes.

So I left.

It wasn't easy. Building an independent practice came with financial risk and uncertainty.

I left because I wanted enough time to listen and understand before I advised. To build a plan that fit the person sitting in front of me instead of the billing rules behind the visit.

That's the practice I have today.

The conversations my patients have now aren't a luxury. They're the kind of care that should have been available all along.
