When a body region is injured, muscles contract protectively. In acute injury, this is adaptive — it limits movement of damaged structures and signals danger to the nervous system. The problem arises when the threat resolves but the protective state doesn't.

Chronic guarding creates a self-sustaining feedback loop. The contracted muscle sends ongoing threat signals to the nervous system. The nervous system maintains the contraction in response. The contraction maintains the signal.

The muscle isn't the source of the problem — it's the symptom of a nervous system that never received the "all clear." This is why treating the muscle alone, without addressing the nervous system, produces limited and temporary results.

In Korean constitutional medicine, Heart–Kidney harmony (心腎相交) describes the body's capacity to move fluidly between engagement and rest — between Yang activation and Yin restoration.

The autonomic nervous system has two branches: sympathetic (fight-or-flight) and parasympathetic (rest-and-digest). Health requires fluid movement between them — stress and recovery, activation and restoration.

Chronic pain changes this balance. It tends to lock the body in a state of sympathetic dominance — persistent low-level threat activation that amplifies pain signals, disrupts sleep, impairs healing, and degrades quality of life in ways that go well beyond the original injury.

Manual therapy works in part by shifting the nervous system toward parasympathetic tone. This is measurable in real time through heart rate variability — and it's a key mechanism behind why skilled bodywork produces effects that outlast the session itself.

This isn't a metaphor. The cycle operates through specific, measurable neurobiological mechanisms — the HPA axis, cortisol dysregulation, impaired glymphatic clearance during fragmented sleep, and inflammatory pathway activation.

Central sensitization is probably the most important — and most poorly explained — concept in chronic pain management.

It describes what happens when the nervous system itself becomes hypersensitive: the volume dial gets turned up and stays up, even after the original injury has long healed. Pain from stimuli that shouldn't cause pain. Widespread tenderness. Pain that no longer maps onto any identifiable tissue damage.

This is not psychological. It is neurological. And it responds — when correctly identified — to the right kind of treatment.

Your body has a built-in analgesic system that most people have never heard of.

The descending inhibitory pathway — centered on a region of the brainstem called the periaqueductal gray (PAG) — releases endogenous opioids, serotonin, and norepinephrine back down into the spinal cord to suppress pain transmission.

Manual therapy activates this system. That's not a hypothesis — it's documented neurophysiology, supported by multiple lines of clinical and imaging evidence.

Understanding this system also explains why how you feel during treatment matters as much as what technically happens. Context, safety, and therapeutic relationship are not soft factors. They are neurophysiology.

The rubbing sends large-fiber sensory signals that compete with and partially suppress the pain signals at the level of the spinal cord. This is Gate Control Theory — first published in 1965 by Melzack and Wall — and it's still one of the most important frameworks in pain science.

Chuna (추나) is a Korean manual therapy system that draws from traditional Korean medicine, Chinese Tuina, American chiropractic and osteopathic medicine, and Japanese manual therapy.

Formalized as a clinical discipline in 1991. Covered under Korea's National Health Insurance since 2019.

This isn't folk medicine dressed up in clinical language. It's a genuine synthesis — and when you look closely at both traditions, you start to notice they've been describing the same phenomena in different vocabularies all along.

The ancient framework and modern neuroscience aren't in conflict. They're converging.
