A successful vascular access procedure doesn't always mean the case is over.

Recently, our team placed a PICC for a patient with extensive bilateral scarring that made vascular access particularly challenging. The line was placed successfully, confirmed, and ready for therapy.

Then, later that evening, a call came into our Vascular Support Team.

A review of post-procedure imaging raised concerns about the catheter position, and the ordering provider requested immediate removal. For many patients, replacing the line would be inconvenient. For this patient, it could have meant a much more difficult path forward.

Before the PICC was removed, an experienced vascular access clinician reviewed the situation and recommended one additional step.

That decision changed the course of the case.

Hospital at Home programs are changing the way many patients receive care, allowing them to continue acute-level treatment in the comfort of their own homes. But they also introduce challenges that don't always exist when resources are just down the hall.

In one of our cases, a patient recovering from sepsis needed laboratory testing to monitor her progress. What should have been a routine blood draw quickly became something else. Despite multiple attempts by another provider, vascular access could not be obtained, and the patient ultimately endured more than 10 needle sticks without the needed samples being collected.

At some point, the question is no longer whether another attempt might be successful. The question becomes whether the approach itself needs to change.

A Vascular Wellness nurse clinician was consulted to assess the situation. What followed not only resolved the immediate challenge, but also highlighted an important lesson for Hospital at Home programs as they continue to expand: when vascular access becomes a barrier to care, early involvement of specialized expertise can have a meaningful impact on both the patient experience and the continuity of treatment.

The full clinical case explores how this story unfolded and why it serves as a valuable reminder that some of the most important clinical decisions happen before the next attempt is made.
