A Skilled Nursing Facility resident arrived in the Emergency Department with a fever of unknown origin and concern for possible sepsis. The sepsis protocol was ready to begin, but there was one major obstacle... none of the facility staff could obtain vascular access.

The patient was severely dehydrated with contracted upper extremities, and multiple traditional and ultrasound-guided IV attempts had already failed by the time our clinician was asked to assess the situation.

In suspected sepsis cases, time matters. Delays in obtaining labs, blood cultures, fluids, and antibiotics can quickly change the trajectory of care.

This case is a reminder that vascular access is never “just a task.” In critical moments, it can become the bridge between uncertainty and intervention.

What made this case especially interesting was the recommendation that followed.

Some members of the care team expected escalation directly to a central line.

A two-month-old infant with RSV and pneumonia had already been through multiple failed vascular access attempts from facility staff when a call was placed to Vascular Wellness.

A Vascular Wellness clinician arrived in the PICU, successfully placed an ultrasound-guided PIV, confirmed it was ready for therapy, and left the hospital.

Then the phone rang again.

The infant had suddenly deteriorated. Additional vascular access was urgently needed, and time was becoming a factor.

The clinician immediately turned around and returned to the hospital.

What happened next became a powerful reminder that in pediatric vascular access, expertise, rapid assessment, and critical thinking can change the trajectory of a situation very quickly.
