Why Oaks Health AfterCare™
As a Doctor of Nursing Practice and Board-Certified Family Nurse Practitioner, I've had the privilege of caring for patients across the continuum of care—from home health nursing and hospital-based care to perioperative and pre-surgical medicine.
Those experiences taught me one important lesson:

Oaks Health AfterCare™ was created to bridge the gap between hospital discharge and your recovery at home by providing personalized, professional support when patients and families often need it most.
As our program grows, our vision is to build a trusted team of healthcare professionals dedicated to delivering exceptional in-home postoperative care while working alongside each patient's surgeon and established postoperative plan.
