Tonight I got a phone call... It wasn't from a patient I treated. It was from a father of a patient who I didnt treat...

Last week, I had a healthy 16 year old athlete walk into the clinic complaining of arm pain after a work out.... had gradually gotten worse over the course of 4 days upto an 8/10.

His left arm was clearly larger than the right (1.25 in at biceps and .75 inch at forearm) his arm was warm to the touch while his fingers were cold. Pulse was barely palpable on the left. I tried to talk myself out of it. But clinical instinct kept telling me this was a significant blood clot, despite being 16 years of age.

I told the patient I didn't feel comfortable treating him, and said its a blood clot until proven otherwise.

Well it wasn't a blood clot it was 3! His condition required a 4 night hospital stay for treatment.

Tonight his father called to tell me they were home and to thank me for my due diligence in taking the time to properly diagnose his son and ensure he got evaluated.

I have students walk out of talks at times when I say the least important things we do is perform manipulation. Its a statement I truly believe though. Yes, manual therapy including manipulation works and is an important part of what we do. But being able to accurately assess a patient and get them the care they need and manage their care is the most important part of what we do.
