And a big shout‑out to the teams from The Livingston Clinic, Livingston Surgical Clinic, and Roe Rehab for helping make the evening run smoothly.
We’re lucky to have such a strong crew.

Forms are the responsibility of the patient/parent. This office does NOT retain copies of completed forms. Replacement forms require a new appointment and additional fee. Don’t forget to turn the form in to your school or coach.

⚠️ This clinic is NOT appropriate for students with heart disease, current symptoms such as chest pain or shortness of breath, or other chronic medical conditions. Please see your primary care provider for a full wellness exam.
