Need to transfer your medical records to follow your provider? We’re here to help patients through that process.

If you haven’t filled out a Release of Information form yet, you can find it here: https://www.sekmhc.org/resources/

Once completed, you can bring the form to any of our offices in person or email it to [email protected] so we can send your records to your provider. Your records are still safely maintained and available during this transition.
