An 82-year-old male was referred to home health after hospitalization for acute on chronic congestive heart failure with preserved ejection fraction. The patient has a long history of hypertension and chronic atrial fibrillation. Hospital records also indicate a history of chronic kidney disease stage 3, hyperlipidemia and a history of smoking ci******es up until he quit a few years ago.
