Herbs for Women Over 40

Sage
Dimpfel and colleagues (Heliyon, 2021) gave 80 women aged 48 to 65 sage extract tablets or placebo for 4 weeks. The sage group's hot flush severity score fell 55.3%, from 15.9 to 7.1, and beat placebo from week 3. One short trial.

Ashwagandha
Gopal and colleagues (J Obstet Gynaecol Res, 2021): 100 women in perimenopause, 91 finished, took 300 mg of root extract twice a day or placebo for 8 weeks. Menopause Rating Scale scores fell more than on placebo. Estradiol, a hormone, rose too, so it is not for everyone.

Red Clover
Kanadys and colleagues (Nutrients, 2021) pooled 8 trials of red clover extract: 1.73 fewer hot flashes a day than placebo. Results varied a lot between trials.

Maca
Brooks and colleagues (Menopause, 2008): 14 postmenopausal women took 3.5 g of maca powder a day for 6 weeks and placebo for 6 weeks. Mood scores on a menopause checklist were better on maca. Hormone levels did not change. Very small.

Fennel
Lee and colleagues (Nutrients, 2020) pooled 12 trials of fennel for period pain. It favored fennel over placebo, but results were very inconsistent and the authors rated the evidence low.

Ginger
Negi and colleagues (Cureus, 2021) pooled 5 placebo trials of ginger for period pain. Pain scores were 2.67 points lower than on placebo, though results varied a lot.

Chamomile
Shabani and colleagues (Open Public Health Journal, 2022) gave 200 students with period pain chamomile sachets (5,000 mg, 3 times a day) or mefenamic acid for 2 cycles. Pain was similar in both groups. No placebo group.

Black Cohosh
Leach and Moore (Cochrane, 2012) reviewed 16 trials with 2,027 women. For hot flushes, black cohosh did no better than placebo.

Vitex
Schellenberg and colleagues (BMJ, 2001): 170 women took 20 mg of agnus castus fruit extract or placebo daily for 3 cycles. 52% versus 24% had their premenstrual symptoms drop by at least half. One trial.

Most studies here are small or mixed, so this is not proof it will work for you. Ask your doctor first if you take medication or are pregnant.
