We are honored to be part of this wonderful community tradition and can’t wait to celebrate alongside all of you. Be sure to watch for us in the parade route, give us a wave, and help us make our first year one to remember!

We are seeking a compassionate Licensed Practical Nurse (LPN) to join our Home Health & Hospice team. In this role, you will provide personalized, skilled nursing care to patients in their homes while supporting patients and families and collaborating with an interdisciplinary care team.
Responsibilities:
• Provide skilled nursing care in patients’ homes
• Monitor patient conditions and communicate changes to the care team
• Administer medications and treatments as prescribed
• Assist with pain and symptom management
• Educate patients and families on medications, treatments, and self-care practices
• Collaborate with healthcare professionals to ensure continuity and quality of care
• Maintain accurate and timely patient documentation
Qualifications:
• Current Wisconsin LPN license
• Valid Wisconsin driver’s license and reliable transportation
• CPR certification
• Home Health and/or Hospice experience preferred
• Strong communication, assessment, and organizational skills.
