What does a joyful, self-determined future after high school look like?

This “Transition to Adulthood” package from THINK+change helps caregivers navigate transition planning one step at a time through practical tools, training, and powerful real-life stories.

A highlight of the package is TALKS – Life Beyond High School, a 15-minute documentary featuring Beck Williers, son of PEAK Parent Center CEO Michele Williers. Through Beck’s story, viewers see what’s possible when resilience, self-advocacy, and strong community support come together to build a meaningful life in adulthood.

The package also includes:
🎓 Training to help families prepare for transition planning in school
🔦 A transition tool map that breaks down transition steps by age and life stage
