Introducing our next session titled: The Clot Thickens: Interactive Anticoagulation Cases, presented by Seferina Kim, PharmD, CACP.

This fast‑paced, case‑based session explores high‑risk anticoagulation scenarios commonly encountered in outpatient practice. Through realistic patient cases and audience participation, attendees will identify medication safety concerns, recognize clinically significant bleeding that requires intervention, and evaluate common challenges involving bridging and transitions of care. The focus is on practical decision‑making, patient safety, and real‑world pharmacist interventions.

About the presenter:
Dr. Seferina Kim is a graduate of the University of Maryland School of Pharmacy and has practiced across multiple specialties including anticoagulation, primary care, nephrology, oncology, and transitional care. She currently serves as clinical staff for the University of Maryland School of Pharmacy with practice sites in the outpatient Medication Therapy Management Clinic and Family Medicine at the University of Maryland Medical Center. Dr. Kim is chair of the UMMC Anticoagulation Stewardship, chair of the UMB/UMBC Intimate Partner Violence Collaborative, and coordinator for the PGY‑2 Ambulatory Care Residency at the University of Maryland School of Pharmacy.

This session will provide the latest updates on novel drugs and biologics, as well as new dosages and formulations of existing medications, with a focus on therapies commonly used in the care of older adults. Attendees will review key clinical changes, mechanisms of action, side effect profiles, and practical considerations such as market availability and cost.

About the presenter:
Dr. Jessica Merrey is the Lead Clinical Pharmacy Specialist in Ambulatory Care at The Johns Hopkins Hospital and is Board Certified in Pharmacotherapy, Ambulatory Care, and Geriatric Pharmacy. She maintains clinical practice sites in Internal Medicine, Geriatrics, and Anticoagulation and has served in national leadership roles within the Board of Pharmacy Specialties.

Executive Director Aliyah Nuri Horton and MPhA President-Elect Eriny Victor recently attended the 2026 NASPA/Pharmacists Mutual Leadership Conference in Rockville!

The conference brought together pharmacy association leaders from across the country for collaboration, learning, and leadership development ahead of the upcoming presidential year. As a proud member of the National Alliance of State Pharmacy Associations (NASPA), MPhA was honored to be part of these important national conversations shaping the future of pharmacy leadership.
