Compassionate care can make all the difference. One patient recently shared how they learned about Welvista’s Medication Assistance Pharmacy through a community partner and received support during a challenging time.

With the help of Community Outreach Advocate Linda Stewart, the patient was able to navigate the application process and access quality healthcare services and medications. For many patients, having someone who takes the time to listen, provide guidance, and offer support can be just as important as the assistance itself.

At MAP, we know that health and stability go hand in hand. That's why partnerships like this matter. By equipping our Community Outreach Advocates with additional resources, we can better connect the people we serve to the support they need, whether that's medication assistance, utility help, or other essential services.

Unable to provide proof of income from occasional or cash-based work? The Self Declaration of Income Form may be the solution.

This form is used when it isn't possible to get proof of income from work such as collecting and selling scrap metal, flea market sales, roadside stands, miscellaneous yard work, or other informal jobs.
