ISP stands for Individual Support Plan. It’s a personalized plan used in services for people with Intellectual and Developmental Disabilities (IDD) to guide the support they receive each day. But more than just a document, an ISP is built around a person’s life—their goals, routines, preferences, and what matters most to them.
Through an ISP, support teams work alongside individuals to help them live the life they choose. That can include building skills, staying connected to family and friends, finding meaningful activities, and growing independence in everyday life. At its core, an ISP reflects how we serve—by listening, adapting, and making sure each person is supported as an individual, not just a diagnosis.
