At Therapeutic Innovations, your privacy and trust are important to me, and I believe you should have a choice in how you complete your intake paperwork.
While I use Vagaro, a professional scheduling and practice management platform with HIPAA-compliant features and data security protections, I also offer traditional paper intake forms for those who prefer them.
Yes, the old-fashioned way! Paper intake forms are kept securely under lock and key, and this option is always available to you.
Simply let me know before your appointment or leave a note when scheduling online, and I’ll be happy to have your paperwork ready.
A little reminder about appointment notifications:
Complimentary text message reminders are provided as a courtesy to help you keep track of your appointments. You’re always welcome to opt out of these notifications if you prefer.
Please remember that clients are responsible for their scheduled appointments, whether or not they receive text reminders. A $50 missed appointment fee applies.
