Primary care is coming to Sage Wellness Birth Collaborative! 🌿

Beginning in August, Nurse Practitioner Gabby with Grounded Family Practice will be joining us two days a week to provide personalized primary care for individuals and families. Gabby's goal is to make it simple for our mama's to have seamless outpatient support after delivery, while also having the option for ongoing primary care.

Sage and Grounded believe that healthcare should be built on relationships, education, prevention, and whole person wellness- not rushed appointments.

Services will include:

✨Pediatric Care
✨Well Woman Care
✨Family Healthcare
✨School & Sports Physicals

You'll enjoy longer visits, personalized care plans, and a provider who takes the time to listen and support your health goals. We're so excited about this partnership and ready to help care for every season of life! More details coming soon!
