Today, we remember. We remember the horrific attack on our country, the brave men and women that responded to the attack, and the grief that surround our lives. At DHCHD, we wanted to honor the loss that our country faced by having a Legacy of Remembrance where we had moments of reflections, memories, and tears when we remember 9/11. As we mourn, we can also celebrate and honor our brave first responders, volunteers, and military that helped with the aftermath of the attack. Today as we remember, we can also come together in community and lead with love❤️🤍💙

As we get older, deciding which Advanced Directive we need for our healthcare needs can be challenging to understand. There are many different types of Advanced Directives that do different things. Texas Health and Human Services provide Advanced Directive forms on their website with no cost to download or use. Some of the forms that are readily available include:
1. Declaration for Mental Health Treatment (DMHT)
2. Directive to Physicians and Family or Surrogates (Living Will)
3. Medical Power of Attorney Designation of Health Care Agent (MPOA)
4. Statutory Durable Power of Attorney (SDPOA)
5. Out-of-Hospital Do-Not-Resuscitate (OOH-DNR) Order

Medical Power of Attorney (MPOA) become effective ONLY when you are no longer capable of making your own decisions. Many well-wishing families and individuals confuse this form and have the belief that once they have an MPOA they are able to make decisions on the behalf of other immediately. This is not the case. Only when someone is incapacitated does the MPOA take effect. FYI: Once you need an MPOA, it is too late to fill one out😉 so get this done before you need it!

If you sign a DNR in the hospital, it will not work in community settings including Nursing Homes and Assisted Living Communities. For community settings, individuals needs an Out-of-Hospital Do-Not-Resuscitate (OOH-DNR) Order signed and filled out correctly to adhere to wishes of not being resusciated.
