We are hiring!!! Home Health Coordinator & VA Team Lead
CareMo Home Health is seeking an organized, proactive, and detail-oriented Home Health Coordinator & VA Team Lead to join our growing team. In this dual-capacity role, you will serve as the primary bridge between our patients, field clinicians, and physicians, while also serving as the Lead for our Virtual Assistant (VA) team, directly overseeing all remote VA operations.
The ideal candidate excels at healthcare scheduling, regulatory compliance, team management, and workflow optimization—ensuring both field clinicians and offshore administrative staff operate efficiently to deliver exceptional patient care.
Key Responsibilities
1. Virtual Assistant (VA) Team Leadership & Operations
Supervise & Lead: Oversee day-to-day operations, task delegation, and productivity for the team of virtual assistants (VAs).
Training & Onboarding: Train new VAs on agency protocols, HIPAA compliance, specialized software (EMRs, communication tools), and workflow processes.
Quality Assurance (QA): Conduct regular performance audits on VA outputs (e.g., medical scribing, chart preparation, phone interactions, and data entry).
Workflow Optimization: Develop standard operating procedures (SOPs) to streamline virtual administrative tasks and resolve operational bottlenecks.
2. Patient Intake & Clinical Coordination
Process incoming patient referrals, verify insurance coverage, and secure prior authorizations.
Conduct initial patient intakes, collect medical histories, and set up electronic health record (EHR) profiles.
Coordinate with attending physicians regarding orders, plan of care (POC) updates, and medical approvals.
3. Clinical Scheduling & Rostering
Schedule field clinicians (RNs, LPNs, Physical Therapists, Home Health Aides) with patients based on geographic location, clinical needs, and language preference.
Manage visit reassignments, cancellations, and staff absences quickly to maintain seamless care coverage.
4. Compliance & Records Management
Track and ensure the timely receipt of signed physician orders, OASIS assessments, and field visit notes.
Enforce strict HIPAA compliance and privacy regulations across both local and remote VA operations.
Key Qualifications & Requirements
Education: Bachelor’s degree Nursing
Experience:
2+ years in home health care coordination, medical scheduling, or healthcare administration.
1+ years of supervisory or team lead experience managing remote/offshore Virtual Assistants.
Technical Skills:
Proficiency with Home Health EHR software, WellSky/Kinnser.
Familiarity with remote management tools , Google Workspace.
Core Competencies: Strong leadership, medical terminology knowledge, problem-solving, and cross-cultural communication skills.
