"Have you tried losing weight?" πŸ‘‡

I know this is a skit. It's still hard for me to watch, because the humor is coming from a real place. The reason it's funny is that so many people have sat in that chair and heard exactly that. It lands because it's true.

🦴 What that pain can actually mean
Pain that starts in the shoulder and travels down the arm is often coming from the neck. A pinched nerve up there can send pain, tingling, or weakness all the way down. Your shoulder might be fine.

βš–οΈ Where weight fits in
Weight can matter for the knees, the hips, the low back. Joints that carry you all day. A nerve in your neck doesn't care what the scale says, and telling someone to lose 30 pounds for a radiating arm pain skips the whole exam.

🩺 What should have happened instead
Questions about where it starts and where it travels. A look at your neck, not just your shoulder. Maybe imaging. Then a plan based on the structure causing it.

Why this matters. If your pain has been handed back to you as "lose weight" more than once, you might not have been diagnosed yet. You've been dismissed.

Diagnosed with knee arthritis? Please read this first πŸ‘‡

Most people leave that appointment thinking they have two options. Take something for the pain, or wait until the knee is bad enough to replace. Here's what I wish more people knew.

🦡 What arthritis actually is
The cartilage cushioning your knee is wearing down. It won't grow back on its own. That part is true. The part people get wrong is assuming surgery has to be the next stop.

πŸ’‰ The cortisone cycle
A shot every few months when it flares. This is where a lot of people get parked. It calms things down for a while, and the cartilage stays exactly as it was.

🎯 The middle ground
Most patients never hear about this one. Image-guided PRP takes your own concentrated platelets and places them into the damaged area, under imaging, so we know we're in the right spot. The goal is to help the tissue itself. I have knee patients whose pain has stayed resolved for over a year after treatment.

Why this matters. If the only two options you were given were pills and a replacement, you were given a short list.

Not all pain is the same πŸ‘‡

The way your pain feels is one of the biggest clues to where it's coming from. Nerve pain, joint pain, and muscle pain each show up differently, and telling them apart is where a diagnosis starts πŸ‘‡

⚑ Nerve pain
Burning and shooting. If that describes your pain, it is typically coming from a nerve.

🦴 Joint pain
A deep ache, or a sharp pain that hits when you move. Stiffness usually goes along with it, so the joint feels tight or limited.

πŸ’ͺ Muscle or tendon pain
Sharp, and it gets worse when you press on it. The pain stays concentrated in one spot instead of spreading out, which typically points to a muscle or a tendon.

These symptoms matter because they help us diagnose the problem and find the actual source of your pain. Treatment only works when it's aimed at the right structure.

A law changed last year right here in Florida, and for some of you it is the difference between managing pain forever and actually repairing what is causing it πŸ‘‡

Licensed Florida physicians can now perform stem cell therapy for orthopedic conditions and chronic pain. Knee arthritis. A hip that has been grinding for years. A shoulder that wakes you up at 3am.

πŸ”¬ What it actually involves
We take stem cells and place them into the damaged part of the joint. Your body then gets help repairing the tissue that is causing the pain. Medication sits on top of the problem and leaves the damage where it is.

🦡 Why this matters
For a long time the plan for a worn out knee was pills, then stronger pills, then a replacement once you could no longer stand it. There is now something real to try in between those.

Hi, I'm Dr. Kelly DeSouza. I started Elite Relief MD because I kept meeting people who were failed by the usual approach. Told there was nothing left to try, or pushed toward surgery before anyone had really looked.

My background is in anesthesiology and interventional pain management. I trained and later taught at the University of Miami, and I'm double board certified in both.

Some pain heals on its own. Some pain is waiting for you to stop hoping it will πŸ‘‡

And here's the part most people get wrong: how MUCH it hurts is not the clue. How it BEHAVES is. From an interventional pain doctor πŸ‘‡

βœ… Behavior that usually points to healing
It's sore after activity, then fades over a day or two. That's tissue recovering.
It's stiff in the morning but loosens once you get moving.
Rest genuinely helps, and the relief holds.
Each week is a little better than the last, even if it's slow.

🚨 Behavior that usually points to something that needs treatment
Weeks or months in, and nothing is trending better.
Rest doesn't help the way it used to, or it hurts even at rest.
It's started traveling, down an arm, down a leg.
Tingling, numbness, or weakness has joined it.
And the big one: what used to help doesn't anymore.

Either way, don't just assume. Pain that worries you is worth getting checked no matter how long you've had it.

Not all pain is the same πŸ‘‡

Muscle, joint, and nerve pain each feel different, and the way yours feels is a clue to where it's coming from πŸ‘‡

πŸ’ͺ Muscle pain
A dull, deep ache you can point right to.
Tender when you press it, sometimes with a tight band or knot. Worse when you use it, better with rest and heat.

🦴 Joint pain
A stiff internal ache that's hard to pinpoint because it comes from deep inside.
Sharp with certain movements, and famous for morning stiffness that loosens as you get going.

⚑ Nerve pain
The unmistakable one. Burning, shooting, electric.
Pins and needles, numbness, and it travels down an arm or a leg, sometimes when you're not even moving.

The tricky part is these can overlap. A joint problem can spasm the muscles around it and pinch a nerve at the same time, so treating the loudest symptom can mean missing the source entirely.

That's why the first thing I do with every patient is find the true source of the pain, confirm it with live imaging, then aim the treatment exactly at it.

Advice I'd give you as an interventional pain doctor if I wasn't worried about hurting your feelings πŸ‘‡

⏳ Waiting is costing you.
Pain gets harder to treat the longer it's been there. Your nervous system learns it, it starts spreading past where it began, and the window where simpler treatments work well closes quietly. The patients who come in early do better than the ones who tough it out for two years.

πŸ’Š More medication is not a plan.
Medication has a role, and it manages a symptom while whatever is generating your pain keeps doing it. If your dose keeps climbing and nobody has looked for the source, that's maintenance rather than treatment.

πŸ₯ Surgery is not the only thing left.
A lot of people are told the operating room is their only option and never get offered anything before it. There's usually a lot to try first, and most of it is far less invasive than what you were told was your last resort.

Can chronic pain actually be cured? Sometimes, yes.

For other patients the win looks different. I find the exact source of the pain, treat it directly, and they get long stretches of real relief.

So a cure is possible. And either way, that was never the whole question.

Pain sits on a scale. Going from an eight to a two changes your entire life, and there is almost always room to move that number. That's the work I do every day as a pain management doctor, whether the answer for you ends up being a full cure or lasting relief that gives you your life back.
