When I hear a baby clicking during a feed, I know the seal is repeatedly breaking. The question then becomes why that baby cannot maintain the seal. Is the tongue able to stay in contact with the palate? Is the TMJ moving symmetrically? Are the temporal bones affecting the position or movement of the jaw? Is the hypoglossal nerve able to support strong, coordinated tongue movement?

The clicking itself is not the diagnosis. It is a clue that helps us know where to look next, and it is one more reason why positioning alone may not resolve the problem. Global Cranial Evaluation teaches you how to follow those clues back to the larger structural pattern.

One baby may present with a wide, flat head and a broad, low palate, while another has a narrow, elongated head and a high, vaulted palate. Both babies may be described as colicky, and both may be struggling with feeding, but that does not mean they have the same structural pattern or need the same clinical approach.

This is where a symptom-based protocol can fall short. We talk so much about the tongue when a baby is struggling to latch, but the tongue does not work alone. The jaw needs to move freely, the palate needs to provide the right surface for contact. All structures have to work together for a baby to latch, suck, seal, and transfer milk effectively.

This is why I teach practitioners to look beyond one structure or one symptom. Clicking, leaking milk, fatigue at the breast, and poor transfer may all be clues pointing to a larger cranial and cervical subluxation pattern. When you can see the whole pattern, you can make much more informed clinical decisions.

You can’t run the same protocol on every colicky baby and expect the same result. One baby needs help getting the tongue into contact with a low, flat palate. Another is already fighting increased pressure under a high vaulted one. Same complaint on paper. Completely different structural reality in the chair.

A baby who cannot sustain a suck long enough to finish a feed gets labeled the same way almost every time. Posterior tongue tie. Refer for frenotomy.

But the hypoglossal canal sits right next to the atlas. Subluxate that joint and the tongue loses strength and coordination. It looks exactly like a tie. It is not a tie. It is a nerve under pressure.
