Clinical Pearl of the Day 💊

Thinking about adding berberine to your routine?

Berberine has real evidence behind it for metabolic health. But in a randomized crossover study in healthy volunteers, 300 mg three times daily for two weeks reduced the activity of three major drug-metabolizing enzymes: CYP2D6, CYP2C9, and CYP3A4.

Translation: berberine can change how your body clears certain medications.

That matters if you take things like:
Statins
Metoprolol
Warfarin or other blood thinners
Tramadol or codeine
Certain antidepressants

And because berberine also lowers blood sugar on its own, stacking it with metformin or other diabetes medications can push glucose lower than you intended.

None of this means berberine is off limits. It means it deserves the same review a prescription would get.

This is exactly what a pharmacist-led medication and supplement review is for. Catching the interaction before it becomes a problem.

Your gut is part of your hormone story.

Most women get told perimenopause is just an o***y problem. But there’s a whole system working alongside it that almost nobody checks.

A few things worth knowing:

Certain gut bacteria decide whether estrogen exits your body or gets recycled back in

Estrogen helps protect your gut lining, so when it drops, digestion often changes too

Anti-inflammatory compounds made by your gut bugs tend to decline after menopause

Sleep, anxiety, and brain fog run through this same gut-brain conversation

The probiotic aisle is not the problem.

Most people start there because it’s the easiest thing to reach for. Take one a day, wait a few weeks, hope the bloating settles down. When it doesn’t, the usual conclusion is that you picked the wrong brand, so you try another one.

But strains are not interchangeable. The one that helps a friend’s digestion may do nothing for the fatigue, food reactions, or skin flares you’re dealing with, because those may be coming from something the supplement was never designed to address.

Guessing gets expensive, and it costs you time you could have spent actually feeling better.

We’d rather start with information: what your symptoms are telling us, what your history looks like, how you’re actually living day to day, and functional testing when it’s warranted.

Your gut is specific. Your plan should be too.
