Located on the Eastern Shore of Maryland, Choptank Community Health System, Inc. (CCHS) is a private, non-profit community health center providing primary health care services in Caroline, Dorchester, and Talbot Counties and the surrounding areas. CCHS health centers provide a full range of primary medical, dental, and behavioral health services. CCHS is a participating provider with most major in

surance companies and HMOs. In addition, a sliding fee scale is available for any patient without insurance. As a community health center, CCHS is able to use federal funding to expand current services and develop new programs to meet the health care needs in our area. Families in will have greater access to pediatric dental care and primary care as Choptank Community Health System expands services at its Chestertown Health Center by adding three new providers to its dental and medical teams.

Leading the expansion is pediatric dentist Beatrice Bautista, DMD, whose arrival significantly increases pediatric dental capacity at the Chestertown Health Center.

She is joined by family nurse practitioners Morgan Bleinberger, DNP, CRNP, FNP-C, and Lauren Burnett, DNP, CRNP, FNP-C, expanding access to comprehensive primary care for children and adults throughout Kent County and the Mid-Shore.

Choptank Community Health System is proud to partner with public schools in Caroline, Dorchester, Kent, Queen Anne’s, and Talbot counties to help make health care more accessible for students throughout the school year.

Our School-Based Health Centers bring health care directly to participating schools, helping students spend more time learning and less time away from the classroom.

The excitement of a new school year is here. Along with shopping for school supplies, meeting teachers, and settling into new routines, Choptank Community Health System encourages you to put your child's health at the top of your back-to-school checklist.

Healthy students are better prepared to learn, grow, and participate in all that school has to offer. Students enrolled in the School-Based Health Center program have convenient access to quality health care right on their school's campus.

As you prepare for the first day of school, here are a few ways to help your child get off to a strong start:
• Enroll your student in the School-Based Health Center if they are not already enrolled. Enrollment is free. Find locations and forms at https://www.choptankhealth.org/schoolbased
• Make sure your child's immunizations are up to date.
• Schedule sports physicals before fall athletic seasons begin.
• Update the School-Based Health Center with any changes to your child's medications, medical history, or emergency contacts.
• Support healthy habits at home by encouraging nutritious meals, regular physical activity, and good sleep. Students enrolled in the program also have access to nutrition counseling and behavioral health services when needed.

Choptank Health's School-Based Health Centers provide comprehensive medical, dental, nutrition, and behavioral health services for enrolled students. Care is coordinated with your child's primary care provider and dentist to ensure they receive the support they need.
