Testosterone isn't just a male hormone.

Women make it too, and it plays a role throughout the body. But Dr. Natkin's new article is clear that the evidence is narrow: the strongest research is for low sexual desire after menopause. For energy, memory, muscle, bone and weight, studies haven't consistently shown a benefit.

Side effects are possible, and one low number on a lab report isn't a diagnosis. If therapy makes sense, the goal is the lowest dose that works, with follow-up labs.

Erectile dysfunction isn't always about testosterone.

Dr. Natkin's new article explains why. Sometimes testosterone is low, but it is only one part of male sexual physiology. Erectile function depends on much more than hormones: the brain, nerves, hormones and blood vessels all have to work together.

A few points from the article:
- Stress and performance anxiety can make an er****on harder to maintain.
- ED can sometimes be an early cardiovascular warning sign.
- Certain medications may contribute to ED in some patients. Don't simply stop a prescribed medication.
- Raising testosterone does not necessarily fix ED. A man's testosterone can be optimized while ED remains.
- Treatment should follow the underlying physiology, and evaluation is individualized.

Vitamin D: hype or reality?

Dr. Natkin's new article looks at what the science really tells us. Vitamin D is essential, but the amount you take is not the same as the level in your blood, and neither is necessarily the same as the amount of active vitamin D your body is making.

A few points from the article:
- Vitamin D3 is a precursor. It is processed by the liver and then the kidneys before it becomes the active hormone.
- Two people taking exactly the same amount can end up with very different blood levels.
- The evidence strongly supports avoiding significant deficiency. It becomes less certain when universal targets of 60-100 ng/mL are recommended.
- Vitamin D may influence telomere biology and autoimmune disease risk, but it has not been proven to slow human aging.
- Correcting deficiency is not necessarily the same thing as optimizing physiology.

Creatine is more than a sports supplement.

Dr. Natkin's new article explains why. Creatine helps tissues rapidly regenerate ATP, the molecule cells use to do energy-dependent work. That matters in muscle, and the same system also operates in other tissues with high energy needs, including the brain and heart.

A few points from the article:
- Creatine does not directly build muscle. It supports rapid ATP regeneration, which can improve training capacity over time.
- During medical weight loss, it may help support the resistance training needed to preserve muscle. It does not prevent muscle loss on its own and cannot replace protein or training.
- With aging, it may be a useful addition to resistance training.
- The evidence for memory and thinking is less established than the evidence for muscle.

Losing weight is not the whole goal.

Dr. Natkin's new article explains why muscle matters during medical weight loss. Whenever the body is in a calorie deficit, some loss of lean tissue can occur along with fat loss. That becomes more important when weight comes off rapidly or when appetite and calorie intake drop a lot.

The goal of a thoughtful medical weight-loss program is better body composition, better metabolic health and preservation of physical function.

A few points from the article:
- Protein should be intentional and spread across the day, even when meals are smaller.
- Resistance training gives the body a reason to keep its muscle. Aerobic exercise still matters too.
- The scale cannot tell you whether you are losing fat, muscle or water. Strength and how you function matter as well.
- More rapid weight loss is not automatically better weight loss.

The better objective: lose excess body fat while preserving muscle, strength, nutrition and long-term metabolic health.

Do longer telomeres mean you're younger? Not necessarily.

Our new article explains why. Telomeres are protective caps at the ends of our chromosomes, and they generally become shorter as we age. But telomere length is influenced by genetics, age, environmental exposures, health conditions, lifestyle and even how the measurement itself is performed.

That is why a single telomere measurement should not be interpreted as a definitive measurement of how quickly, or how well, you are aging. Biological aging involves many systems, and telomere length is only one component of that larger picture.

Exercise, avoiding to***co, sleeping adequately and eating a nutrient-dense diet already have substantial health benefits, independent of what happens to your telomeres.

Men need estrogen too.

Dr. Natkin's new article explains why. Estradiol is the primary biologically active estrogen in men, and it plays important roles in bone health, sexual function, body composition and cardiovascular physiology.

On testosterone therapy (TRT), a portion of testosterone is naturally converted into estradiol by an enzyme called aromatase. When testosterone levels increase, estradiol may increase as well. That increase is not automatically a problem. The more important question is whether the level is appropriate for the individual patient and whether it is associated with clinically meaningful symptoms.

Estradiol can also be too low. Extremely low estradiol is not the goal of testosterone therapy. The objective is hormonal balance, not estrogen elimination. And an elevated lab result should not automatically lead to another medication.

The point: treat the patient, not just the number.
