HEALTHeLINK is putting the data to work for the WNY Integrated Care Collaborative. As part of a collaborative project, we are working with WNYICC to utilize HIE data to evaluate early results of the social care network and the impact it has had on regional health improvement, specifically with Medicaid members.

In running the region’s social care network, WNYICC and its partners have so far screened more than 65,000 Medicaid members across Erie, Niagara, Cattaraugus and Chautauqua counties, said Nikki Kmicinski, CEO of WNYICC.
