Standing in Italian Parliament this week felt different.

Not just a meeting. A turning point.

In a room filled with leaders across governance, education, policy, and healthcare, I presented the Theory of Predictive Emergence and demonstrated ATLAS live. Not theory. Not projections. Real time.

In less than one week, we have surpassed 1000 patient assessments from every continent.

This is real world evidence as it happens. Global, transparent, on demand, and built to give back to the communities where patients live.

For the first time, we are not just observing adherence. We are seeing it emerge.

And in that room, I said it clearly:

“Il paziente non è il problema.”

The patient is not the problem.

It holds true in any language because it points to the same truth. When adherence breaks down, it is a system misalignment.

What made this moment even more significant is what happened beyond the stage.

Conversations turned into alignment. Alignment turned into partnerships. The kind that do not stay local.

Benvenuto a Roma! I am here just landed in beautiful Italy where I will meet with the Italian Senate and esteemed colleagues in the next evening in support of the promotion of therapeutic adherence initiatives ahead of World Adherence Day. In parallel, I will also host my own LinkedIn Live event on Friday for the adherence project 2026, where I will engage with colleagues around the world asking how they map, measure and manage adherence in their own communities.

The weather is so nice and the people are so welcoming. I am looking forward to these important discussions and how predictive emergence can contribute to identifying fertile behaviors earlier in the process.

Not a Dose. A Duration.

At 00:00 GMT, the window for The Adherence Project 2026 officially opened and the counter reset.

For the next seven days, we’re collecting something that is usually talked about in theory but rarely observed in real time, what adherence actually looks like across time, across countries, across real lives.

Last year, the theme was Action over Awareness. That idea still stands. Awareness alone has never changed outcomes. But this year asks a harder question.

What happens after the first day? After the second? After the week where everything goes right?

Because a single dose is easy to capture. Even a short stretch of perfect behavior can look convincing on paper. But adherence has never been about isolated moments. It’s about what holds when routine sets in, when life gets busy, when no one is watching.

That is what we are measuring this week.

Not intention, not recall, not what we hope is happening. We are looking at patterns. At consistency. At behavior sustained across time.

Time is not something to adjust for in the background. It is the signal.

Right now, submissions are already coming in from around the world. Each one is a real person, a real regimen, a real pattern that contributes to a dataset that researchers, pharmacists, and institutions will actually use.

If you’re on any kind of daily regimen, take two minutes and see where you land. If you work with patients, ask one person today. Not as a check, just as a conversation.

There’s a quiet moment right before something meaningful begins.

Not loud. Not announced. Just a shift you can feel.

We’re a little over a day away from the Adherence Project collection window resetting from testing and opening to the public. What started as controlled validation, careful iteration, and respect for the science is now ready to meet the real world.

This has never been about measuring behavior in isolation. It’s about recognizing adherence as a state, something dynamic, something influenced, something that can be understood and improved with intention.

I have spent the entire weekend refining the engine behind it: ATLAS.
ATLAS was built to measure something that has always been difficult to capture in real time: behavior itself. Not pharmacy refill proxies. Not pill counts. Not retrospective estimates. Actual participation from people, across places, at the moment behavior occurs.

For decades, adherence research has relied on indirect signals. Useful, but incomplete. What we have lacked is a system capable of observing patterns as they emerge across geography, populations, and time. That is what ATLAS is designed to do.

A platform where individuals can participate directly.
A framework where data appears in real time.
A living map of adherence behavior unfolding across the world.

If the experiment works, it will represent something new:

a global research opportunity open to anyone, anywhere.

A distributed study of behavioral stability powered not by institutions alone, but by participation. The machine is ready. Now it needs people.
