Not gait training? High intensity still matters 🔥

Elevating heart rate can help prime the nervous system for learning and support neuroplasticity.

The challenge? Many functional tasks don’t naturally get patients into higher intensity zones, especially for those who are non-ambulatory. 

Consider using conditioning-based interventions to elevate HR, then immediately transition into task-specific practice.

💡Example: raise HR with rope slams → practice transfers.

Intensity + task specificity = a powerful combination for driving functional change 👏

Planks aren’t just for abs 🔥 They’re a functional movement that can help neurological patients improve core stability, posture, balance, coordination, and overall body awareness. A strong core creates a stronger foundation for everyday movements like walking, transferring, reaching, and standing safely.

The best part? Planks can be modified for every ability level
✨ Wall planks for beginners or decreased weight bearing
✨ Elevated planks on a bench or table for more support
✨ Knee planks to reduce load while building stability
✨ Forearm or high planks depending on wrist/shoulder tolerance
✨ Dynamic variations to challenge coordination and control

Curtsy lunges are a great way to target the hip abductors while challenging single-leg stability and reducing knee valgus collapse during functional tasks like stair climbing and squatting.

Added the tidal tank for increased perturbations, reactive core engagement, and dynamic balance demands 👏

6 WAYS TO REDUCE RETROPULSION! ✨

Retropulsion during transfers can make functional mobility feel unsafe and unpredictable — but targeted intervention can improve anterior weight shifting, confidence, and motor control 👏

Here are some of my favorite ways to combat retropulsion during transfers:

1️⃣ Anterior ball roll outs with lift off
➡️ Promotes forward weight shift, trunk activation, and controlled center of mass movement.

2️⃣ Tall kneel squats (add resistance if appropriate)
➡️ Challenges postural control while strengthening hips and core in a functional position.

3️⃣ Anterior weight shifts with hip extension off wall
➡️ Encourages loading into the forefoot while facilitating hip extension and upright posture.

4️⃣ Decline wedge under feet
➡️ Forces anterior tibial translation to improve forward momentum and reduce posterior bias.

5️⃣ Incline wedge under feet using error augmentation
➡️ Creates an overcorrection strategy to help the nervous system recalibrate forward weight shifting.

6️⃣ Resisted sit-to-stands
➡️ Builds strength, momentum generation, and confidence with transitional movement patterns.

Small changes in positioning and task setup can make a huge difference in transfer success 💡
