Hi, I'm Dr. Sandra Koehn! 👋

I'm a board certified internal medicine physician and founder of Planted Direct Primary Care, a small practice in Rochester built around one simple idea: you deserve a doctor who actually knows you.

After years in a system that rewards speed and volume, I saw patients wait weeks for sick visits, get just minutes in the exam room, see a different face each time, and leave with unanswered questions. I knew there had to be a better way to practice medicine. So I built one.

Planted DPC is intentionally small, capped at 200 patients. You'll have unrushed visits, direct access to me by phone or text, and continuity with the same doctor. No insurance middleman for routine care, no per-visit fees, just one transparent monthly membership.

I provide whole-person care, including prevention and cancer screening, lifestyle and nutrition support, weight loss, and care for high blood pressure, high cholesterol, and prediabetes. As a DO, I also offer osteopathic manipulative treatment for pain and tension.

Most of all, I want to listen, connect the dots, and never make you feel rushed or like a number. You take care of everyone else. I'd be honored to take care of you.

Have you ever wondered what an OMT visit is actually like? 🌿

OMT, or osteopathic manipulative treatment, is hands-on care. I use my hands to assess and treat muscles, joints, and connective tissue, with the visit centered around you.

Here’s what you can expect

1️⃣ We talk first about what’s bothering you and what your body has been through.

2️⃣ You stay fully clothed and comfortable.

3️⃣ I use gentle pressure and positioning techniques that release tension from connective tissue (fascia) and rebalance muscle tension, restoring motion, symmetry and function. Nothing is forced or cracked without warning.

4️⃣ We talk about movement, posture, and daily habits that may support you at home.

Many people feel lighter or looser afterward. Others notice a change over the next day or two. OMT may be helpful for back or neck pain, tension headaches, jaw or sinus pressure, and that tight, heavy feeling from carrying kids, bags, and stress all day.

It does not work the same way for everyone, and it does not replace other care you may need.

Anyone can schedule a visit - you don't have to be a patient! Non-members can book an OMT appointment too. If you’ve been carrying everything, you deserve care that makes room to listen. 🌱

Planted DPC has been open for six months!! 🌱🎉 Time sure flies when you LOVE what you do and are building something great!

Taking the leap of faith to open my own practice was one of the biggest decisions I have ever made. Never in a million years did I ever think I would open my own practice but here I am, 6 months later, thriving and so are my patients!

To everyone as trusted me, and every person who encouraged and supported me along the way, thank you! To my husband who is my biggest fan and support, could NOT have done this without you! To my family, who have contributed time, ideas, and all their many skills, thank you!

To my patients: thank you for allowing me the privilege of caring for you and for building this special place with me, where care is about knowing you and your whole story.

I truly couldn’t be happier that I took this leap. I’m grateful beyond words, and I give all praise to the Lord. 💚

I actually entered residency planning to be a hospitalist: or maybe even dive into critical care. I loved the medicine and the high stakes. 🏥
