We don't have a standard order size. We have a standard.

A single-location clinic reordering a few boxes of one item gets quoted the same way as a health system placing a standing truckload order across a dozen categories: priced honestly, quoted same day, documented the same way.

Volume still earns better pricing. That's just math, and it holds whether the volume is one site or forty. But volume was never the price of entry. Nobody has to round an order up to hit someone else's minimum, and nobody has to wait on a truckload process to move a partial case.

The cheapest week to buy test kits is the week nobody’s sick.

Respiratory season runs the same script every year. First cold snap. Positivity starts climbing. The wrong substitute costs more than the shortage.

Here’s how it usually goes. Your primary product lands on backorder, allocation, or recall. Somebody offers an “equivalent.” It ships, it arrives, and your clinical team rejects it on sight: wrong mil, wrong cuff, wrong adhesive, wrong size run. Now you’ve burned three weeks, a purchase order, and a little bit of trust, and you still don’t have product.

Substitution is a discipline, not a swap. “Equivalent” is a claim. It has to be proven.

The Keystone substitution standard:

1. Match the spec sheet, not the category. Same grade, same material, same critical dimensions, same performance ratings. If the primary says exam grade, 4 mil, chemo-rated, the substitute says it too, in writing.

2. Samples before the purchase order. Your clinical or end-user team approves the product in hand before a single case ships.

3. Documentation carries over. FDA registration, certifications, and spec sheets on the substitute, exactly as you’d get on the primary.

4. Price the switch honestly. If the substitute costs less, you see it. A shortage is not a pricing opportunity.

5. Plan the switch back. We monitor the market year round. When your primary returns to availability, you hear it from us, and the choice is yours. The sub is a bridge, not a trap.

The best supply chain is a straight line.

Manufacturer, Keystone, end user. That’s the path we build toward on every account, because every stop you remove is time and money returned to the buyer.

But healthcare supply doesn’t always allow a straight line, and anyone who pretends otherwise hasn’t worked a hospital contract. Sometimes the path has to bend. A diversity partner holds a seat in the chain. An incumbent distributor is already on file. A shortage puts the product on allocation, and it moves through whoever holds it.

And sometimes it’s the authorization gap. Distributor A is authorized by the manufacturer but has no contract with the hospital. Distributor B holds the hospital contract but isn’t authorized by the manufacturer. Neither one can serve that hospital alone. Together, they can. That gap is where real distributor partnerships are born.

Here’s the part most people get wrong: an extra tier on paper doesn’t have to mean an extra tier in freight. Paperwork can hop. Pallets shouldn’t.
