Please review before applying:

CO2 LASER RESURFACING ~ Flexible dates
• Face, neck, body, scars, or stretch marks
• Commitment to a package of 3 treatments required
• Prior consultation and follow-up after the final treatment required
• Photos used for marketing/educational purposes
• Must be current patient
• Model fee applies

DERMAL FILLER ~ November 14th
• Facial balancing, areas determined with patient/provider and trainer consultation
• Prior consultation and post-treatment follow-up required
• Photos used for marketing/educational purposes
• Must be current patient
• Model fee applies

HOW TO APPLY:

Email [email protected] with front and side profile photos in natural lighting, without makeup. For body treatments, include a photo of the area of interest.

*Please use CO2 or FILLER in the subject line to indicate which opportunity you’re interested in.
