In a PATCH trial secondary study, TXA 1 g bolus + 1 g drip over 8 hrs paradoxically increased complement activation at 24 hours after injury. Delayed increase in complement may reflect the dominant plasminogen activator and raises questions about optimal TXA dosing regimen.

Most of us learned COPD as a clean, simple concept in school.

Real life isn’t clean.

COPD patients don’t fit into neat categories. They exist along a spectrum of disease that includes elements of bronchitis, emphysema, and sometimes asthma.

That matters because your treatment should change based on what’s actually driving their distress.

A few reminders worth revisiting:

You’re not curing anything in the field
Your job is reducing work of breathing and improving ventilation
Wheezing is not your endpoint
NIV used early can prevent deterioration
