Guideline-based lung protection provides the foundation for ARDS care, but ventilation requires further individualization to patient physiology and response. Driving pressure, recruitability, gas exchange and hemodynamics can guide tidal volume and PEEP. During assisted ventilation, respiratory drive and effort require systematic monitoring, while obesity and ECMO may require different interpretations of the same measurements.
