Anxiety, Beyond Coping: The Functional Medicine Path to True Healing

Anxiety can feel like an invisible weight — pulling your energy, robbing your focus, and leaving you feeling on edge even when life looks “fine” from the outside.

Most people are told anxiety is either a personality trait or a simple chemical imbalance in the brain. But after decades in clinical practice, I can tell you: anxiety is rarely “just in your head.”

In functional medicine, anxiety is often the final expression of multiple imbalances in your body — imbalances that no one may have ever looked for.

The Iceberg We Call Anxiety
Anxiety is like an iceberg: the part you see — the racing thoughts, tight chest, sleepless nights — is only the tip. Beneath the surface are layers of biology, environment, and lifestyle that shape your brain chemistry and stress response.

Conventional approaches focus almost exclusively on the visible tip: therapy, medication, maybe relaxation exercises. While those can help, if the systems underneath remain off balance, anxiety keeps resurfacing.

Questions Most People Have Never Been Asked
When I work with patients, I ask questions that make them pause and think, “Why has no one asked me this before?”

The questions below are just a short sample of the kind I ask when working with patients.

As a functional medicine practitioner, I go far beyond these — exploring dozens of interconnected systems, nutrient levels, lifestyle factors, and genetic influences that many standard evaluations never address.

This list is simply a glimpse into the deeper investigative process that helps uncover the true drivers of anxiety:

Has anyone ever measured how your brain's calming and stimulating chemicals are actually balanced — instead of just assuming?

Could the way your blood sugar rises and falls during the day be quietly triggering anxiety spikes?

What's your vitamin D level — and is it in the optimal range for brain health, not just the “normal” range?

Two Often-Overlooked Functional Causes of Anxiety
While there are many biological drivers that can contribute to anxiety, here are two functional medicine factors that often surprise patients.

The following are just two examples of many biological factors that can quietly drive anxiety. In a functional medicine workup, we investigate a much broader set of potential triggers — often uncovering ones patients have never considered.

In my practice, I've identified over 20 biological and environmental drivers of anxiety.

Most patients have never been tested for even half of them.

1. Blood Sugar Instability

Your brain depends on a steady supply of glucose. When blood sugar spikes and crashes throughout the day — from skipping meals, high-carb snacks, excessive caffeine, or processed foods — your body releases stress hormones like adrenaline and cortisol to restore balance.

Those hormonal surges can mimic or trigger anxiety symptoms: racing heart, shaky hands, restlessness, irritability, and sudden waves of unease that seem to come from nowhere.

2. Vitamin D Deficiency

Vitamin D is critical for brain health and mood regulation. It helps regulate serotonin pathways, supports nerve communication, and reduces inflammation in the nervous system.

Even “normal” lab results may not be optimal — and optimal levels matter when it comes to mental health. Suboptimal vitamin D status can magnify stress sensitivity and emotional volatility.

Sarah's Story: From Daily Dread to Deep Calm
Sarah, a 34-year-old teacher, had been battling anxiety for years. She'd tried medication, yoga, cutting caffeine — nothing lasted. She believed she was “born anxious.”

When we looked deeper, we discovered:

Her gut was inflamed and sending constant “distress signals” to her brain.

She lacked key nutrients needed to make GABA — the neurotransmitter that helps the brain relax.

Her cortisol pattern was upside down: high at night, low in the morning.

We built a plan to:

Why These Causes Are Often Missed in Conventional Care
It's not that traditional physicians can't find these causes — many can and do, especially if they've pursued additional training. But the way the healthcare system is structured makes it less likely.

Most primary care visits are short — often 10–15 minutes — and are designed to rule out dangerous conditions, provide symptom relief, and follow established treatment guidelines. In that model, lab testing focuses on detecting diagnosable diseases, not subtle imbalances. If your results fall within “normal” ranges, they're usually considered fine, even if they're not optimal for your body.

Functional medicine works differently. We spend more time (often 45–90 minutes) looking for patterns across multiple systems, using advanced testing when needed.

We aim for optimal ranges, not just “normal,” and consider nutrition, environment, hormones, gut health, and other factors as potential drivers of anxiety.

The two approaches are complementary: traditional medicine excels at rapid diagnosis and safety, while functional medicine dives deeper into the “why” behind persistent symptoms.

Bacteria Linked to Atherosclerosis In The Least Expected Place

Medical literature is very clear that oral dental health plays a significant role in your cardiovascular health.

As odd as it sounds, regularly brushing and flossing your teeth could be what stands between you and a future heart attack.

Research has linked bacteria from the Streptococcus genus to fatty plaque buildup in heart arteries.

A new study published in the journal Circulation has pinpointed two specific oral microorganisms that play a role in whether we develop atherosclerosis (clogged arteries).

In all people there should be a healthy balance of good and bad bacteria in our mouth (teeth). However when this balance is upset, it kicks off a chain of events that makes atherosclerosis far more likely to occur.

The researchers used advanced technology for the study to reexamine the gut and oral bacteria assessments and cardiac imaging from 8,973 volunteers in the earlier SCAPIS study.

Search in the medical literature has actually identified three bacteria associated with atherosclerosis when they were found in large numbers in the mouth. They include:

Streptococcus anginosus
Streptococcus oralis
Streptococcus mutans
The scientists who have published their research have two theories about why these bacteria might be a contributing factor for atherosclerosis.

1:These bacteria (streptococcus species) set off systemic low-grade inflammation that leads to plaque accumulation in heart arteries.

2: In an attempt to evade our immune system, the bacteria in our mouth enter our bloodstream and set up shop in our arteries.

Steps to take to decrease the potential of Streptococcus genus compromising our cardiovascular health include:

Regularly brushing your teeth
Flossing
Consider using a waterpik water flosser (this is my dentist strongest recommendations to clean out bacteria deep in the back molars.) This oral health appliance provides a stream of pulsating water to floss between your teeth and below the gumline.
Don't forget the most important step of ideal dental health and that is regular dental cleanings.
Here are some additional recommendations to be certain your oral health has the best opportunity to be bacteria free.

Stop the Sugar
Salt Water Rinse
Tea Tree Oil and/or Oregano oil products. These essential oils have natural antibacterial qualities. If you battle gingivitis, look for mouthwash or toothpaste made with tea tree oil (never use straight oil) and/or Oregano oil.
Olive Leaf Extract: Olive leaf extract is another natural antibacterial that can be used in mouthwash or toothpaste.

Fats help you absorb all of the beneficial fat-soluble vitamins in plant foods, and some fats have been shown to reduce the risk of heart disease, type 2 diabetes, and obesity.

The key is to eat the right fats and stay clear of the wrong fats. My favorites are avocado, olive oil, nuts and seeds, grass-fed meat, and wild fatty fish.

Chronic Constipation and Cognitive Decline (Early Dementia)--What Is The Connection?

New studies are beginning to see a distinct correlation between poor gut health especially chronic constipation and early dementia.

Chronic constipation may not only be an indicator of gut health, but a potential warning sign of cognitive decline.

Researchers found that among more than 110,000 middle-aged and older U.S. adults, those who were chronically constipated who had fewer than three bowel movements a week showed early signs of poor brain health.

The findings presented at a meeting of the Alzheimer's Association in Amsterdam showed that people with chronic constipation typically performed worse on tests of memory and thinking,

In addition, 73% of these same people stated that were more likely to say their cognitive skills were waning.

Although the study is strictly preliminary and did not absolutely prove that constipation caused the aging brain to deteriorate faster, the researchers of this study do believe there is evidence connecting gut health to brain health.

The researchers theorize that constipation and cognition are linked via the gut microbiome.

The science is exploding with research linking abnormal gut microbiome and various diseases, including degenerative brain diseases like Alzheimer's.

Claire Sexton, senior director of scientific programs and outreach for the Alzheimer's Association stated the it is unclear at this point whether constipation itself or the underlying cause of constipation,that being disruptions in the gut microbiome, is driving this association.

Dr. D**g Wang of Brigham and Women's Hospital and Harvard Medical School and the senior researcher on the study stressed the importance of clinicians discussing gut health, especially constipation, with their older patients.

Of special interest the researched found that people with constipation and worse cognition tended to have relatively few gut bacteria that produced butyrate -- an important fatty acid that helps control inflammation.

I strongly would encourage any and all people suffering with chronic constipation to have a functional medicine or integrative practitioner to do deep dive into their gut health via a comprehensive stool test.

I say why wait for science to catch up with may indeed be a potential connection between compromised gut health poor cognition.
