Join us at Town Square Bergen County for an upcoming educational program focused on Coordination of Care Across Multiple Medical Providers for Older Adults.

This free program from ACAPcommunity will explore how healthcare providers, patients, and families can work together to improve communication, coordinate care plans, manage medications, and create smoother care transitions.

The program will also be available online via livestream. Registration is required.
