FACIAL ENHANCEMENT @
Dr. Hal London

Faces age similarly in most patients. Facial aging involves both the skin and deeper structures.

The skin naturally loses elasticity with age and this is accelerated by sun exposure. Skin aging is manifested primarily by wrinkling and pigment changes. Aging skin tends to sag with the downward pull of gravity. One very overlooked contributing factor is motion from expression. Look at anyone over age 50 and you will see the characteristic lines caused by expression. Frown lines between the brows, accordion lines on the cheeks from smiling, and Christmas tree patterns on the chin from lower face expressions.

I treat skin changes with prescription medication, mainly tretinoin, and resurfacing using laser, peels, and microneedling. Expression lines are the hardest to treat because of their habitual nature. Most patients make voluntary expressions but can’t stop them no matter how hard they try. Fortunately, there are areas of the face where toxins like Botox or Dysport can effectively stop the motion of expression without making you look wierd. I can then apply my restorative treatments without continued skin damage from excessive movement.

Under the skin are the supporting structures of bone, fat, and connective tissues. With age these supporting structures degrade. Bone is lost, fat pads shrink/ fall and cutaneous ligaments loosen. Losing these supportive structures causes the face to sink and sag in predictable patterns.

I treat this structural volume loss with FDA approved fillers. The skill comes in knowing how the face ages and where to inject the filler to restore natural looking contours. It takes years of experience to do it safely and effectively. Like almost every process, go to someone who has at least 10 years of doing it to get your best result.

FACIAL REJUVENATION -Dr. Hal London

A lesson on facial shape, lines around the mouth, and lip contour.

It’s another example of how I strategically place dermal fillers to enhance the face.

With before and after photos you can often see the improvement but not know what has changed. Please allow me to show you. First, knowledge of how the face ages.

Please take a moment to study the before and after photos. Notice the “shape” of her face before treatment. Do you see that before treatment, her face has a more “square” shape, and after treatment, her face appears more heart-shaped? That’s filler in the mid-face.

The “heart-shaped face” is desirable and signifies youth. Why? Faces change shape with age. They sag from volume loss( bone and fat) and skin laxity, subtly changing facial shape over the years. This can be demonstrated by what I call the triangle theory.

Imagine inverting a triangle so that the base( widest part) is now at the bottom instead of at the cheekbones. You can see this demonstrated in the photo presented today.
Adding filler in the mid-face restores the triangle so that the base is again at the top. Nice.


The mouth

It’s an area that ages faster than other areas of the face. Movement is the culprit here, and most of it is voluntary. Talking, eating, smoking, drinking from straws, and expressions all crease skin and cause lines over time. “Smoker’s lines” can be seen in non-smokers too. Some of these movements can be controlled voluntarily through behavior modification , and I help my patients work through that.

Combine excessive movement with volume loss, and you see the perioral area age prematurely. Add sun damage, and the problem multiplies.

Lips

Lips deflate like a tire when they lose fat. What you probably did not know is that you can control how your lips age.

The lower lip- It is especially vulnerable to aging because of facial expression. What I see clinically is that the bottom lip in many patients over age 50 is "smushed" by the pressure of the chin and other muscles. These muscles press the lips together and flatten them. It’s a habitual, voluntary motion that can be stopped with great effort. Habits are hard to break, but just being aware is the first step. Stop"smushing" please.

Please examine the photo before treatment and notice how the volume of her lower lip appears now as a bulge seen just below the red border of the lip( arrows). Enhancing the lower lip with filler masks this feature providing a more youthful contour. You can see what a small evolving bruise looks like on her left lower lip; it's no big deal.

The upper lip- It loses shape more than volume, and restoring shape, especially the Cupid's bow of the central upper lip, is a beautiful addition to any lip enhancement.

Injecting fillers involves considerable technical skill and artistry. There’s a lot of thought that must go into the process before the needle ever touches the skin. Who you go to really does matter.

LOWER FACE ENHANCEMENTS- Dr. Hal London at London Medical Spa

She had typical age-related changes with special concern for her lower face.

What are the lower face targets in most patients? Below the nose, I often enhance lips, mouth corners, marionette lines, and chin. This combination instantly takes years from the face.

Some swelling and bruising are expected but usually minimal. Results last a year or more.

BEFORE AND AFTER PHOTOS

Try this: enlarge the before-photo if you are able and cover just the marionette lines( folds on both sides from mouth to chin) with your fingers. Can you see how age is removed instantly with just this one treatment?

She is not smiling in the after-photo. She just looks more pleasant. Magic: It’s filler in the mid-face and mouth corners. This gives the appearance of lift. Nice.

Lower lip : had that commonly found buldge below the lower lip. Like so many woman, she would press her lips together with expression and speaking. She “smooshed” her lips flat without knowing it. Notice the improvement in the after-photo. Lovely.

Upper lip: her lip had lost volume and shape and was losing the battle with all the lower lip “ smooshing”. Some sculpting of the Cupid’s bow and gentle volume laterally did the trick.
Lips are now more symmetric and just full enough. Beautiful.

SINGLE SESSION ENHANCEMENT- Dr. Hal London

I would estimate the split is 50/50 between those patients of mine who want to get it all done in one session vs those who want or need to do it in split sessions.

I’m fine piecing it together but the overall results might be different if the treatments are spaced too far apart. Allow me to explain.

Enhancing the mid-face, primarily the cheeks and nasolabial folds, has an immediate effect (improvement) on the lower face. That’s why I always start in the midface. It’s possible that less enhancement will be needed in the lower face once the mid-face is addressed. Same situation for the under eyes.

That said, not every patient can afford to enhance the upper,mid, and lower face at one session and I totally understand that. I can make it work very well piecing it together as long as the time between sessions is not too long. That way the overall effect will be global enhancement, not piecemeal improvements.

LASER RESURFACING- Dr. Hal London

Dark spots on the skin are common. They are most often solar lentigo like you see in the photos today. These are caused by sun exposure.

Why do people get more dark spots and wrinkles on the left side of the face? ( keep reading, answer is found at the end)

There are a few ways I can lift pigment from the skin. Fade creams help but take weeks to months to be effective. I prefer mechanical means using acid peels or my CO2 laser.
In addition to removing dark spots, an aggressive CO2 laser treatment improves fine lines and wrinkles and has a skin tightening effect, especially under the eyes.

I had two laser patients this week. Some doctors wont do a laser in the spring and summer. I have no problem doing laser treatment this time of year as long as my patient is compliant with sun avoidance for a week or so. I spend considerable time discussing how sun not only causes the pigment but that maintenance sun protection will keep the spots from reappearing.

The procedure takes about 20 min after 45 min of numbing cream and anesthetic nerve blocks to make the procedure tolerable.
You will be swollen and red for a few days followed by a pink tone to the skin for a week or two.

I love my CO2 laser. It is a powerful tool for skin rejuvenation and no other practice on Amelia Island has one.

BEFORE AND AFTER- 11 years apart- Dr. Hal London

These photos were taken at her first visit and then after her 11th year of regular treatments by me. She comes in only once or twice a year and appreciates the subtle changes I provide.

I am pleasantly surprised by the longevity of these treatments. When she comes in we are not starting from scratch, instead I am restoring prior gains without making her look over done.
