Prior to your appointment we will send you a link to complete a Good Faith Exam or GFE. It is a face-to-face telehealth appointment that takes less than 5 minutes and you will be cleared for up to one year. This will be a one time $30 charge at your visit.

A Good Faith Exam allows a licensed medical provider to review your health history, discuss your goals, and determine whether services such as IV therapy, weight-loss management, or aesthetic treatments are safe and appropriate for you. It establishes the provider-patient relationship and creates an individualized treatment plan before care begins.
