Headaches and Migraines: The Neck-Spine Connection and How Chiropractic Care Can Help
If headaches have become something you simply expect to deal with, it’s easy to fall into a routine: feel one coming on, reach for medication, push through the day, and hope tomorrow is better.
But when headaches keep returning, there may be value in asking another question: Could the way your neck is moving and functioning be part of the problem?
For some headache sufferers, it can be. The joints, muscles, and nerves of the upper neck have close connections with the pain pathways involved in certain headaches. That’s one reason chiropractors pay close attention to the neck when evaluating patients who experience recurring head pain.
One Word, Many Different Kinds of Headaches
We tend to use the word “headache” as though everyone is experiencing the same thing. They’re not.
A tension-type headache may show up after a long day at the computer, a stressful week, too much time looking down at a phone, or hours behind the wheel. Instead of sharp pain, many people describe an uncomfortable squeezing or aching sensation. Tightness through the neck and shoulders can occur at the same time, which is one reason those areas deserve attention when these headaches become frequent.
Then there are cervicogenic headaches, which are headaches linked to a problem originating in the neck. Interestingly, you may feel most of the discomfort in your head rather than your neck. Pain can sometimes travel upward from the upper neck and may be associated with stiffness or difficulty comfortably turning your head. An old neck injury, repetitive strain, or spending hours in one position may also be relevant.
Migraine is another matter. It’s a neurological disorder, not simply a bad tension headache. An attack can be intense enough to interrupt normal activities and may come with nausea, changes in vision, or an unusual sensitivity to light and sound. Neck discomfort is also common in people with migraine, but that doesn’t mean a neck problem is necessarily causing the migraine.
That’s an important distinction. Chiropractic care shouldn’t be presented as a cure for migraine. However, when someone with migraines also has ongoing neck stiffness, restricted movement, or muscle tension, a chiropractic evaluation can help identify whether there are neck-related problems worth addressing.
Why the Neck Can Matter
Think about what you ask of your neck every day. It supports your head while you work, drive, exercise, sleep, read, and spend time on phones and computers.
Hours of repetitive posture, previous injuries, tight muscles, and joints that have lost some of their normal mobility can leave the neck stiff and irritated. Sometimes you notice that as neck pain. Other times, the symptoms may be felt somewhere else.
The upper neck is especially interesting because pain signals coming from this area interact with nerve pathways that also carry sensation from parts of the head and face. That helps explain why a problem beginning in the neck can sometimes be experienced as a headache.
This is where a chiropractic examination can be especially helpful. Rather than assuming every recurring headache has the same source, a chiropractor can look at how well your neck moves, areas of stiffness or tenderness, muscle tightness, posture, and other physical findings. If problems involving the neck appear to be part of the picture, that information can help guide an appropriate approach to care.
How Chiropractic Care May Help
When neck dysfunction is involved, chiropractic care focuses on improving movement and function rather than simply covering up the discomfort.
Depending on what an examination reveals, care may include chiropractic adjustments or other forms of joint mobilization, along with approaches aimed at tight muscles, mobility, posture, and everyday habits that repeatedly stress the neck.
There’s meaningful research supporting this approach for certain headache types. A 2026 clinical practice guideline on chiropractic care for adults with cervicogenic and tension-type headaches reviewed a large body of existing evidence and included spinal manipulation among the recommended non-drug approaches for appropriate patients. Earlier systematic-review evidence has also reported improvements in pain, headache frequency, and headache-related disability following spinal manipulation for cervicogenic headaches.
The evidence surrounding migraine is less straightforward, and it’s important to make that distinction. Migraine is complex and often requires a broader management approach. But when neck pain, restricted motion, or muscle tension accompanies migraine, addressing those physical problems may still be worthwhile.
Looking Beyond the Medicine Cabinet
Headache and migraine medications can be valuable and, for many people, are an important part of managing symptoms. But if your usual response to another headache is always reaching for medication, it may also be worth investigating whether there are physical factors that haven’t been addressed.
Chiropractic provides a conservative, non-drug option for appropriate headache sufferers. When problems with neck movement, irritated joints, or muscle tension are contributing to symptoms, improving those areas may help some people manage their headaches without relying exclusively on medication for relief.
Medication should never be stopped or changed without discussing it with the healthcare professional managing it.
When a Headache Needs Prompt Attention
A familiar recurring headache is one thing. A headache that is sudden, unusually severe, or dramatically different from what you normally experience is another.
Prompt medical attention is appropriate when a severe or unusual headache occurs with symptoms such as weakness, numbness, confusion, fainting, difficulty speaking, major vision changes, fever with a stiff neck, or following a significant head injury.
For headaches that keep returning—particularly when they seem to travel with neck stiffness, soreness, restricted movement, or long periods spent in the same posture—the neck deserves a closer look.
Stop Planning Your Life Around Headaches
Maybe you already know which activities tend to trigger a bad day. Maybe you keep pain relievers nearby or find yourself rubbing your neck and temples while waiting for the discomfort to pass. When this happens often enough, headaches can begin to feel like a normal part of life.
Recurring headaches shouldn’t simply be ignored.
