A “normal” pulmonary function test doesn't always mean your breathing is normal.

I recently saw a patient who smoked 3 packs of ci******es a day, had shortness of breath, chronic cough and wheezing—and felt better with an inhaler.

His PFT did not meet the traditional criteria for COPD.

So what do you do with that?

You look at the whole patient, not just one test.

Symptoms. Smoking history. Physical exam. PFT findings. Air trapping. Exercise tolerance. Response to treatment.

A test is a tool—not the entire diagnosis.

If you're struggling to breathe but have been told, “Your lung tests are normal,” don't automatically assume there's nothing going on.

The clinical picture matters.

Your oxygen level is 98%.

So your lungs must be healthy… right?

Not necessarily.

A normal pulse oximeter reading tells you how much oxygen is in your blood at that moment. It doesn't tell the whole story about how well your lungs are functioning.

If you're getting unusually short of breath, struggling with stairs, stopping frequently when walking, coughing, or wheezing, don't let a “normal” oxygen number convince you that everything is fine.

Your symptoms matter.

98% oxygen is reassuring—but it's not a lung health certificate.

As physicians, we're increasingly recognizing social isolation as a modifiable risk factor — one that deserves the same attention we give to diet, exercise, and smoking cessation in preventive care.
