Hospice is a medically directed interdisciplinary program of health care and services designated to meet the special needs of Patients and their loved ones, when a patient is diagnosed with a terminal illness of which all curative measures has been exhausted and comfort and palliative care is desired. Anyone that has a life threatening illness and a life expectancy of 6 months or less if the disea

se follows its normal course. The patient or the representative chose comfort care instead of curative or aggressive treatment, which is no longer effective or appropriate. The attending physicians, patients, family members, friends, clergy, social workers, discharge planners or any person concerned with patients well being can make a referral. All we need is an order from the primary care physician and we will start the process of determining the eligibility. Who pays for Hospice?

Who provides the care? Care is delivered by a team of professionals who address the physical, emotional and spiritual needs of the patients and their loved ones. The team consists of the patients attending physician, hospice medical director, licensed nurses, hospice aides, dieticians, volunteers and therapist if appropriate.
