Is Your Loved One Ready for a Smaller, More Personal Residential Home?

At Transitional Adult Care, we specialize in serving adults with developmental disabilities who thrive in a calm, low-stimulation environment with individualized support.

Our home may be a good fit for adults who:
• Come from a loving and involved family.
• Benefit from frequent prompting and encouragement with personal hygiene and daily routines.
• Need assistance with activities of daily living while maintaining as much independence as possible.
• Prefer a structured, predictable environment.
• Have low behavioral support needs and do well in a peaceful home setting.
• Benefit from compassionate, patient caregivers who understand that every individual deserves dignity, respect, and consistency.

We understand that choosing residential care is one of the most difficult decisions a family can make. Our commitment is to provide the same level of respect, compassion, and individualized attention that we would want for our own family members.

If you’re exploring residential options for your loved one or would like to learn more about our home, we welcome the opportunity to connect with you. Please feel free to message us privately for more information or to discuss whether our home may be the right fit.

Because the right home isn’t just about availability—it’s about finding the right match for everyone involved.

We found Nemo! Today our residents created their very own mini underwater worlds, complete with colorful gravel, plants, and everyone’s favorite little clownfish. Watching each person build, create, and proudly show off their finished aquarium reminded us that it’s the simple moments that bring the biggest smiles.

Hands-on activities like this encourage creativity, independence, sensory exploration, and conversation—all while having fun together.

At Transitional AFC, we’re not just providing care… we’re creating experiences and memories.

🐠 We found Nemo… and made a home for him! 💙

🎆 Fireworks, creativity, and lots of smiles! ❤️🎨

Today’s art activity was a blast! Using paint, glitter, and a little imagination, our artists created their own fireworks masterpieces. ✨

There is something so special about watching creativity come to life. Every piece is unique, every brushstroke tells a story, and every accomplishment deserves to be celebrated.

At Transitional AFC, we believe art is more than just making something beautiful—it’s about building confidence, encouraging self-expression, and creating joyful moments together. 💙
