The new name — Polyendocrine Metabolic Ovarian Syndrome — matters because this condition was never just about “cysts.”

PCOS has a new name: PMOS. And this matters.

For decades, PCOS has been misunderstood, underdiagnosed, and undertreated.

The Lancet has now published a global consensus to rename PCOS as PMOS: Polyendocrine Metabolic Ovarian Syndrome — a name that finally reflects what this condition really is: a complex endocrine-metabolic disorder, not simply an “ovarian cyst” problem.

PMOS can affect insulin resistance, weight, cycle regularity, ovulation, fertility, acne, hair growth, mood, and long-term metabolic health.

Yesterday was my season opener at the Kinetic Olympic Triathlon — and what a day! 3rd place Masters Overall!!!

Great race, great energy, and a powerful reminder of what consistent fitness efforts can build over time.

Like everyone else, I’ve had to balance training with real-life priorities: family, clinic, work, fatigue, schedule changes, and the endless list of responsibilities that come with adulthood. I’m incredibly grateful to my coach for helping me squeeze every ounce out of every minute of training and make the most of the time I had.

I’m also deeply thankful for the love and support from my family, our clinic team, friends, and so many of our patients who continue to encourage me along the way. It means more than you know.
My hope is that this inspires someone to keep going.

Fitness does not happen overnight. It takes patience, consistency, frustration, setbacks, and a lot of days where you show up even when it is not perfect. But over time, those efforts compound. They can change what you believe is possible. They can help you feel stronger, more capable, and more alive as you age.

It’s been a little quiet on our page lately, but behind the scenes we’ve been working hard on some exciting things at Ultimate Wellness.

We’ve been building, refining, and continuing to grow the clinic in ways that better support our patients and the long-term vision of what physician-led, evidence-based care should look like.

At the same time, Dr. Giffi has been putting in the work on his own health and performance goals too. Last week included strong rides, solid run work, and another reminder that real progress is rarely about one perfect workout. It is about consistency, week after week, when nobody is watching.

One of the best parts of last week was seeing that same pattern reflected in so many of our patients. Better energy. Better habits. Better fitness. Better nutrition. Better momentum. Real change does not usually happen from one big moment. It comes from repeated effort, better choices, and staying the course long enough to let those choices add up.

That is what we believe in here. Not quick fixes. Not hype. Just a real plan, and the power of consistency over time.

Estradiol Patch Shortage?

This Isn’t a Shortage. It’s a Shift.

For years, women were told:

“It’s just aging.”
“Your labs are normal.”
“Hot flashes are part of life.”
“Take an SSRI.”

Now something interesting is happening.

Estradiol patches are on back order in many pharmacies.

Not because estrogen is dangerous.
