Learning is my love language! And if you are my clients, you know that our sessions are full of random surprises and facts.

On January 11th, I start my PhD in Counseling and Psychological Studies. This has been on my bucket list for years, and I refused to settle for a program that did not match the work I actually do, so I kept looking until I found the right one.

I have been building toward this one filled notebook at a time. Fifteen years of handwritten notes, conferences, case reviews, and questions I could not stop asking. Because I keep landing on the same thing.

People do not struggle with their blood sugar because nobody told them what to eat. They struggle because life gets loud, most guidelines are black and white, and at some point they stop feeling like a whole person and start feeling like a lab value.

So that is what I am going to study. Purpose, identity, and meaning in chronic disease care, and what we are missing when we leave those out.

This degree is not a pivot. It is the icing on a cake I have been baking for fifteen years, and it is going straight back into how I serve my clients.

Your 6 week postpartum visit is usually about two things: 1. when you can have s*x again and what you’re doing for birth control.

But if you had gestational diabetes, add one more line to that list.

Ask for the oral glucose tolerance test.

Gestational diabetes didn’t just come and go. It told you something. Women who’ve had it carry about a 10 times higher risk of type 2 diabetes, and when we actually test moms a few weeks after delivery, as many as 1 in 3 already have blood sugars that aren’t normal.

The American Diabetes Association and American College of Obstetricians and Gynecologists both recommend that test between 4 and 12 weeks postpartum. The sad part is that only 1 in 3 are tested.

So say it out loud at that visit: “I had gestational diabetes. I want the two hour glucose test.”

And if fasting in a lab with a newborn isn’t realistic, ask for an A1C or a fasting glucose instead. Something is better than doing nothing.

Most heart attacks happen below the knee.

A woman’s hormonal life from her first period all the way through menopause changes cardiovascular health risk.

Estrogen is cardioprotective. It helps keep blood vessels flexible, supports the lining of your arteries, and helps hold LDL cholesterol down.

As estrogen declines through perimenopause and menopause, you lose some of that protection. LDL tends to rise, body fat shifts toward the middle, and insulin resistance often increases.

The plaque narrowing the arteries feeding your heart is narrowing the arteries feeding your feet at the same time.

In diabetes, the small vessels below the knee are often where it shows up first, sometimes years before anything happens in your chest.

Diabetes can also quiet the nerves in your feet, so the usual warning sign of poor circulation, aching legs when you walk, may never arrive. The first clue ends up being a sore that will not close.

At your next appointment, ask for a PAD screen. Pulses in both feet, skin color when your leg is raised and when it hangs down, and whether your legs get tired when you walk. It takes about two minutes.

A few things that help when the hunger just isn’t there:

1. Anchor meals to the clock, not your appetite. First meal within an hour of waking, then every 4 to 5 hours.

2. Lead with protein. Aim for 25 to 30 grams a meal to protect your muscle. Eggs, Greek yogurt, chicken, a protein smoothie.

3. When a full plate feels like too much, make it count. Greek yogurt with berries and nuts beats skipping breakfast.

4. Keep easy backups on hand for no-appetite days, like a protein shake or a rotisserie chicken.

The goal was never just lower numbers. It was a stronger, healthier you, and that takes fuel.
