When patients leave the hospital, what is the biggest failure in their care? π₯
It might be simpler than you think. A study published in BMJ Open looked at more than 3,700 adults to see how often discharge communication breaks down.
Researchers looked at three basic things patients need when leaving:
β Written instructions π
β Clear discussion of medications π
β A follow-up plan ποΈ
The results? 17.1% of patients (about 1 in 6) reported poor communication. But the biggest gap of all was follow-up care. In fact, 23% of people said no follow-up arrangements were made for them at all!
This means the problem isnβt just confusing language - itβs patients leaving without knowing their next step. Even worse, patients with poor mental health and social service needs were the most likely to experience these communication gaps.
