TRT & Blood Donation: Why Phlebotomy Shouldn't Be Your Only Tool 🩸
Are you on Testosterone Replacement Therapy (TRT) and constantly being told to donate blood when your hematocrit goes up? We might need a Plan B.

Here is what often gets missed:

It is a temporary fix: Donating blood lowers your hematocrit for a few minutes, but testosterone continues to stimulate red blood cell production, so it will climb right back up.

It drains your energy: Every donation strips your body of iron. Repeated donations can severely deplete your ferritin and iron stores, leaving you completely exhausted.

Phlebotomy shouldn't be the only tool in the toolbox to manage the hematologic effects of TRT. If you are donating regularly, make sure your provider is checking your ferritin and full iron levels, not just your hematocrit!

"Normal" Labs but Still Exhausted? You Might Be Iron Deficient 🩸
Iron deficiency is missed all the time, often because providers either don't check it or they rely on flawed lab ranges.

Here is why you might feel like you got hit by a small bus even if your labs aren't flagged:

"Normal" doesn't mean optimal: Your ferritin (stored iron) might come back at 18, and because the bottom of the lab range is 10 or 15, it won't be flagged as abnormal.

Hemoglobin is a late indicator: Hemoglobin can be a late sign of iron deficiency. You can completely deplete your iron stores long before your hemoglobin ever drops and you actually become anemic.

Ignored symptoms: Even with "normal" labs, you might be totally exhausted, have brain fog, feel dizzy when you stand up, experience terrible workouts, or notice hair loss.

You shouldn't have to be your own part-time medical detective. In my practice, I generally want to see a patient's ferritin above 60, and sometimes closer to 100.

The Takeaway: Be persistent and ask your provider for a full iron panel and ferritin check. Because "not flagged" doesn't automatically mean everything is okay, and labs need to be actually interpreted, not just given a green checkmark!

Myth vs. Reality: Testosterone in Women 🧬

It’s a natural hormone: Testosterone is a normal and essential female hormone, not just for men!

Balance is the goal: Hormone therapy aims to restore healthy female levels, not push you into male ranges.

Know the red flags: While mild oily skin can happen, a deep voice or masculinization are not typical. Those are signs of excessive dosing.

The answer isn't to fear testosterone. The key is working with a practitioner who will dose it appropriately, monitor your levels, and actually listen to you. Women need testosterone, too!

Don't just accept "everything is normal" on your lab results. 🛑

Next time you get a CBC blood test, check your MCV (red blood cell size). If it's high, your provider might be missing a B12 or folate deficiency, an underlying thyroid issue, or the effects of regular alcohol use.

I love AI and use it every day, but here’s the truth: AI doesn't have 20 years of medical experience. You can feed it all your labs and history, and it can still confidently give you the wrong answer.

Why? Because medicine requires clinical judgment—knowing when labs are wrong, recognizing complex patterns, and seeing the big picture.

Absolutely use AI to research and ask your provider better questions! But remember: The future isn't AI replacing doctors. It’s great doctors who know how to use AI. 🩺✨
