Due to illness, our provider is out of the office today. All scheduled appointments will be rescheduled, and our staff will be contacting patients to arrange a new appointment time.

Our office staff will be available until 12:00 PM today to answer calls, return messages, and assist with any questions or scheduling needs.

To all my patient with recent labs drawn —->if you receive a bill from Labcorp please call them - I DO NOT BILL FOR THEM!!!!

They send bills before sending to insurance for some reason?? Please call them and verify ur insurance and make sure they send it to ur plan! This information is sent to them when drawing labs but they still do it!!??

Reach out to Labcorp Patient Billing at 800-845-6167, available Monday through Friday, 8:00 a.m. to 5:00 p.m. ET, for assistance.

Please leave a voice mail if you need any medication refills—-IN ORDER to get those meds refilled- leave the NAME OF THE MEDICATION AND DOSE and the pharmacy you wish to have it sent to! THANK YOU!!

Please be advised that our office does not complete or process DMV Medical Examination forms for drug or alcohol related issues for any state outside of Louisiana. This policy applies to all out-of-state (non-Louisiana) requests without exception.
