Sciatica can be frustrating, especially when pain, tightness, or tingling keeps coming back.

Sometimes, the problem isn't just coming from your back. The sciatic nerve can also become irritated or restricted as it travels through the surrounding muscles and tissues.

Here's a look at some of the hands-on work we do to help improve how the nerve moves and reduce irritation.

The goal is simple: help you move better and get back to doing what you enjoy.

A great start to October.

I spent the first two days of the month doing hands-on training, learning, and talking through cases with Dr. Cody Scharf at Thrive Care.

Even after more than 25 years in practice, I’m still looking for ways to get better at evaluating and treating difficult muscle, joint, and nerve problems.

A big part of that is helping patients better understand what’s going on, so they can feel less afraid of their condition or injury and more confident about moving forward.

Thanks to Dr. Scharf and the Thrive Care team for a great couple of days.

The location of your pain is a clue—but it does not always reveal the cause.

Two people may both point to the same painful area, yet the reason for their symptoms may be completely different.

One person may have a joint that is not moving well. Another may have soft-tissue adhesion limiting movement. Someone else may have an irritated nerve or a nerve that is not moving normally.

That is why treating everyone with the same symptom in the same way does not make sense.

A diagnosis can give the problem a name, but it does not always identify the specific restriction that needs attention.

Good care starts with a careful evaluation of the individual—not simply treating the place that hurts.

Inspiration may help us begin, but consistent effort is what moves us forward.

Behind every skill, accomplishment, and meaningful result are the hours most people never see—the practice, patience, mistakes, and willingness to keep improving.

This Labor Day, we recognize the people whose daily effort helps build, support, and care for our communities.

Your hard work matters.

Still stretching the same “tight” area with little or no lasting change?

That may be a sign that the problem is not simply a muscle that needs to be stretched more.

Ongoing tightness can have several possible causes:

• Soft tissue that is not moving normally
• A joint that is not moving well
• A nerve that is irritated or restricted
• An activity that repeatedly overloads the area

Stretching can be helpful when it matches the problem. But repeatedly stretching without knowing what is causing the restriction may provide only temporary relief—or no relief at all.

If the same tightness keeps returning, the better question may be:

Why does this area keep tightening?

A thorough evaluation can help identify what is actually limiting your movement and whether treatment is appropriate.

Still tight after stretching?

Stretching can feel good but if the tightness keeps returning, the muscle may not be the whole problem.

Restricted movement can involve several factors, including:

• A joint that is not moving well
• Irritated or restricted nerves
• Soft-tissue adhesions
• Weakness or poor movement control
• More activity than the area can currently tolerate

That is why repeatedly stretching the same spot may provide only temporary relief.

The better question is not, “What should I stretch more?”

It is, “Why does this area keep feeling tight?”

At Discover Soft Tissue & Spine, we evaluate how your muscles, joints, soft tissue, and nerves are functioning before deciding what treatment may help.

If the same tightness keeps returning, the problem may need a closer look.

How do you know whether your treatment is actually helping?

Pain relief matters but feeling better for a few hours or days does not always mean the underlying problem is improving.

Progress may also look like:

• Moving more freely
• Doing more with less discomfort
• Returning to activities you have been avoiding
• Maintaining improvement longer between visits

That is why we regularly recheck movement and function not just ask, “How do you feel today?”

Treatment should not continue automatically because another visit is on the schedule. There should be a clear reason for what is being treated and measurable evidence that it is helping.

If progress has stalled, the answer may not be more of the same. It may be time to reassess the problem and adjust the plan.

Can’t touch your toes?

That does not automatically mean your hamstrings are tight—or that something is wrong.

Touching your toes requires coordinated movement from your hips, pelvis, low back, muscles, and nerves. Your body proportions and the way you perform the movement also matter.

If one area is not moving well, another area may compensate. That is why repeatedly stretching your hamstrings may produce only temporary improvement—or no noticeable change at all.

The better question is not simply, “How can I stretch farther?”

It is: “What is actually limiting this movement?”

At Discover Soft Tissue & Spine, we evaluate how your joints, muscles, soft tissue, and nerves are moving before deciding what may help.

If recurring tightness, stiffness, or restricted movement keeps returning, it may be time for a closer look.

Times are changing and so is the way we practice.

We’re not practicing the same way we did 30 years ago. We continually refine our examination and treatment skills as new information and better methods become available.

Last weekend, Dr. Lambert traveled to Cedar Rapids, Iowa, for additional hands-on training with Thrive Care. The goal is simple: keep learning, keep improving, and provide care based on what we know today—not what we knew decades ago.

Experience matters. So does continuing to grow.
